“Coming out” as a parent of a gay child

[M]y elder son David was fifteen when he told us he was gay – not that he had actually intended to tell us quite then.

He said he was meeting someone but was evasive as to who this might be? I forced the issue never expecting to hear that this was some guy he had met on line through a gay website.

Alarm bells rung at the possible danger!

David must have guessed we might find the news of him being gay difficult as he kept repeating, “It’s OK Mum, there’s nothing wrong”.

My husband’s first thought was “I love my son. I don’t want to lose my relationship with him”.

As for me, I have an unfortunate knack of sometimes putting my big feet in things.

Whilst reeling from the shock, thankfully I avoided saying anything that my son would feel hurt or rejected by.

We both understood that what mattered most was for David to stay believing in himself and to know that our love and support was unconditional.

David appreciated the way we had accepted his sexuality and to stop us feeling anxious, he agreed to cancel the internet date.

Having “come out” to his friends and immediate family, David visibly looked happier by the day.

Now the ball was in our court. Was it our turn to “come out” as parents of a gay son? Would that be fair to David? Was it for him to decide who and when to tell others or not? At the young age of fifteen, we felt it was. That made it much harder because I wanted to feel accepted too.

Up to the point when David told us he was gay, I had no knowledge or experience of what being LGBT+ meant.

My head was full of fears which were further fuelled when I went on-line and came across far right materials discounting LGBT+ as wrong and blaming being gay on abuse or an unhealthy mother-son relationship.

Was I a bad Mum? I feared being judged. I was worried now how David would be treated. Would his school teachers who had praised him as a role model now think less of him?

Would he find himself rejected as unsuitable to be an RSY Summer Camp Leader?

Having brought my boys up to feel strongly Jewish, I now felt anxious that this might not sit comfortably with fully accepting and supporting David’s sexuality.

My Jewishness is all bound up in family and home, celebrating Friday night and all the family traditions. So for validation and support, I turned to my Jewish roots. As I said, I wasn’t ready to “come out” publicly and so like my son before he “came out”, I turned to the privacy of the internet for help. I tapped into Google “Jewish Mum of gay son” and up came “Parents of Jewish Gays and Lesbians” with a number you could phone in confidence.

Going for the first time to the group “Parents of Jewish Gays and Lesbians”, I was scared as to quite who I would find there.

The wonderful thing was how unbelievably just like the two of us the other parents all were. They could have come straight out of any Shul – parents anxious to do right by their children. We were no longer on our own.

Hearing from other parents and sharing our own story in a Jewish group in which we felt understood and accepted, helped us feel better. The first pernicious lie it immediately destroyed for me was the idea that being gay had anything to do with upbringing or by extension anything I had done or not done. It was a fact of life, period.

A Dad said that the last thing he would ever wish on his son would be to be imprisoned in an unhappy marriage hiding his sexuality. That hit home and made me rethink the dream I had been nurturing of one day seeing my son under the Chuppah with grandchildren to follow. My son had his own life to lead. I just wanted him to be happy and true to himself. And so in the group we parents chatted on into the night. We discussed why it was that so many of our LGBT+ children were going to Shul less? Did our LGBT+ children no longer feel they could count themselves as proper members of the club?

Perhaps like me before I became aware of LGBT+, our kids assumed by default that within Shul life their sexuality was taboo and that they would not be understood or accepted unless they hid their sexuality.

To be fair, if I joined any club, I would want to feel that there was someone there a bit like me and that I wasn’t just going to be tolerated, but actually wanted by the club.

My journey has been much easier than for some as being of my own making – struggling with my own prejudices. Thankfully the positive attitude of both our Shul and my son’s school explains why David has never felt ashamed of his sexuality and why both his friends and our Shul friends when told have had no issues.

In the twilight zone before feeling ready to come out to the world as a Jewish parent of a gay child, it helps to share feelings in the trust of absolute confidentiality with likeminded parent souls who understand. I am now Co-Ordinator for the parents’ group, “Parents of Jewish Gays and Lesbians” which helped me so much and which I would like to see there for other parents.

It is a really important group not just for the parents but also for LGBT+ children as “happy parents make happy kids”. Unfortunately the group is hardly known about so if you get a chance to tell others about the group, I would ask you to please do so.

Researchers Reveal an Evolutionary Basis for the Female Orgasm

Though a common occurrence (hopefully), the female orgasm has been a biological mystery.

[F]ew things are as magical as the female orgasm, whether you are experiencing it, inducing it, or just a casual observer. It is essentially pure art in motion. Yet, there are many things we don’t know about the phenomenon, scientifically speaking, such as, why it exists. Scientists have been pondering this for centuries.

Apart from vestigial organs, there are few structures in the body we don’t know the function of. It seems that the clitoris is there merely for pleasure. But would evolution invest so much in such a fanciful aim? Over the years, dozens of theories have been posited and hotly debated.

One prevailing theory is the “byproduct hypothesis.” The penis gives pleasure in order to drive males toward intercourse and ensure the perpetuation of the species. The sex organs are one of the last things developed in utero. Due to this, and the fact that women develop their pleasure organ from the same physical structure the penis is formed from, the clitoris is therefore a “byproduct” of the penis. You could imagine how some women feel about this theory.

Another is the mate-choice hypothesis. Here, it is thought that since a woman take longer to “get there,” it would pay for her to find a mate invested in her pleasure. A considerate lover would make a good father, the theory posits. Yet, the female orgasm happens rarely during penetrative intercourse, undercutting this theory.

It’s been thought that the act plays a role in conception. Several studies have shown that the woman having an orgasm during intercourse increases the likelihood of impregnation. But how and why is not well understood. Now, a team of scientists suggest that the female climax once played a role in reproduction, by triggering ovulation.

Researchers at Yale University posed this theory, in a study published in the Journal of Experimental Zoology Part B Molecular and Developmental Evolution. Gunter Wagner was its co-author. He is a professor of ecology and evolutionary biology at the university. According to him, previous research has been looking in the wrong place. It focused on how human biology itself changed over time.

Instead, these Yale researchers began by analyzing a large swath of species and the mechanisms present in females associated with reproduction. Wagner and colleagues also looked at the genitalia of placental mammals. They focused on two hormones released during penetrative intercourse across species, prolactin and oxytocin.

Prolactin is responsible for the processes surrounding breast-milk and breast feeding, while oxytocin is the “calm and cuddle” hormone. It helps us to bond and feel closer to others. Placental mammals in the wild need these two hormones to trigger ovulation. Without them, the process cannot occur.

One major insight researchers found is that in other species, mammalian ovulation is induced by contact with males, whereas in humans and other primates, it is an automatic process operating outside of sexual activity, called spontaneous ovulation. From here, they looked at those female mammals who induce ovulation through sexual contact with males. In those species, the clitoris is located inside the vagina.

Evolutionary biologists believe that spontaneous ovulation first occurred, in the common ancestor of primates and rodents, around 75 million years ago.  From here, Wagner and colleagues deduced that the female orgasm must have been an important part of reproduction in early humans. Before spontaneous ovulation, the human clitoris may have been placed inside the vagina. Stimulation of the clitoris during intercourse would trigger the release of prolactin and oxytocin, which would in turn, induce ovulation. This process became obsolete once spontaneous ovulation made it onto the scene.

“It is important to stress that it didn’t look like the human female orgasm looks like now,” said Mihaela Pavličev, Wagner’s co-author of this study. “Homologous traits in different species are often difficult to identify, as they can change substantially in the course of evolution.” She added, “We think the hormonal surge characterizes a trait that we know as female orgasm in humans. This insight enabled us to trace the evolution of the trait across species.”

While the hypothesis is compelling, it has drawbacks. The biggest is that it’s difficult, if not impossible, at least currently, to investigate what, if any, sexual pleasure other female animals derive during copulation. Other experts say, more data is needed from other organisms to shore up this theory. Still, it seems the most persuasive argument to date.

Why queer history?

[F]ifteen years ago, as a junior scholar, I was advised not to publish my first book on the persecution of gay men in Germany. And now, one of the major journals in the field has devoted an entire special issue to the theme of queering German history. We have come a long way in recognising the merits of the history of sexuality–and same-sex sexuality by extension–as integral to the study of family, community, citizenship, and human rights. LGBT History Month provides a moment of reflection about struggles past and present affecting the LGBT communities. But it also allows us a moment to think collectively, as a discipline, about the methods and practices of history-making that have opened space to new lines of inquiry, rendering new historical actors visible in the process. In asking the question “why queer history? ” not only do we think about how we got here and the merits of doing this kind of work, but we question, too, whether such recuperative approaches always lead to more expansive, inclusive history. In other words, to queer history is not just to add more people to the historical record, it is a methodological engagement with how knowledge over the past is generated in the first place.

The great social movements of the 20th century created conditions for new kinds of historical claims making as working and indigenous people, women, and people of colour demanded that their stories be told. Social history, and later the cultural turn, provided the tools for the job. Guided by a politics of inclusivity, this first wave of analyses by scholars like the extraordinary John Boswell searched out evidence of a historical gay and lesbian identity–even marriage–in the early modern and medieval period. Michel Foucault’s History of Sexuality vol. 3 would fundamentally alter the playing field, as he questioned the veracity of such quests, arguing that it said far more about our contemporary need for redress than about history itself. Modern homosexual identity–he instructed historians –first emerged in the 19th century through the rise of modern medical and legal mechanisms of regulation and control. The discipline was turned on its head. Instead of detail-rich studies of friendship, “marriage”, and kinship a whole new subfield emerged focused around the penal code, policing, and deviance. In the process of unmasking the mechanisms of power that circumscribed the life of the homosexual, lost from view was the history of pleasure, of love, and even of lust. Although providing a much-needed critique of homophobic institutions, the result was a disproportionate concentration on the coercive modernity of the contemporary age.

And yet, despite these pitfalls, the Foucauldian turn introduced much-needed interdisciplinarity into historical analyses of same-sex practices. Of those who took up the challenge of a critical history of sexuality that sidestepped the pitfalls of finding a fully formed pre-modern identity were medievalists and early modernists keen on questions of periodization and temporality, basically how people in past societies held distinct ways of knowing and being what it meant to live outside the norm. If Foucault had fundamentally destabilised how we understood normalcy and deviance, these scholars wanted to take the discussion further still, to interrogate how the experience of time itself reflected the presumptions and experiences of the heteronormative life course.

By queering history, we move beyond what Laura Doan has called out as the field’s genealogical mooring towards a methodology that might even be used to study non-sexuality topics because of the emphasis on self-reflexivity and critique of overly simplistic, often binary, analyses. A queered history questions claims to a singular, linear march of time and universal experience and points out the unconscious ways in which progressive narrative arcs often seep into our analyses. To queer the past is to view it skeptically, to pull apart its constitutive pieces and analyse them from a variety of perspectives, taking nothing for granted.

This special issue on “Queering German History” picks up here. Keenly attuned to how power manifests as a subject of study in its own right as well as something we reproduce despite our best intentions to right past wrongs, a queer methodology emphasises overlap, contingency, competing forces, and complexity. It asks us to linger over our own assumptions and interrogate the role they play in the past we seek out and recreate in our own writing. To queer history, then, is to think about how even our best efforts of historical restitution might inadvertently circumscribe what is, in fact, discernible in the past despite attempts to make visible alternative ways of being in the world in the present.

Such concerns have profound implications for how we write our histories going forward. Whereas it was once difficult to countenance that LGBT lives might take their rightful place in the canon, the question we still have to account for is whose lives remain obscure while others acquire much-needed attention? While we celebrate how far we’ve come–and it is a huge victory, to be sure–let us not forget there still remains much work to be done.

The way we teach sex-ed is old and ineffective. Here’s how to fix it.

[I]n a predictable bit of news, the results of a study released this past September show that students consider most sex-education programs to be out-of-touch, outdated, and lacking in the information that might actually prove useful to them. Among the deficiencies reported by teenagers were a focus on fear-based lesson plans, curricula that alienate LGBTQ+ students, instructors untrained in actually providing useful sex-ed, and a failure to acknowledge that some young people are  —  spoiler alert  —  sexually active.

When it comes down to it, though, these inadequacies do not stem from lack of trying on the part of certified sexuality educators. There are disparities in curricula, and in resources: Federal funding for sex-education flows to both abstinence-only and evidence-based approaches, and decisions about curricula are made on a state-by-state  —  and district-by-district  —  basis. There are still only 13 states that require sex-education to be “medically accurate.”

In fact, in the past year, 23 bills were introduced with the intention of restricting the quality of sex-ed. Such restrictions included moves to limit access to information about reproductive health options, and to exclude qualified sexuality educators from schools based upon their affiliation with abortion providers.

While the majority of these bills failed to advance, in many cases, educators continue to be hamstrung by red tape. And they worry that  —  in the wake of the most recent presidential election  —  their jobs will only become more difficult. What is an enterprising, conscientious sex-educator to do?

Recently, I attended the National sex-ed Conference in Atlantic City, New Jersey, where I saw sexuality educator Francisco Ramirez present a keynote on “hacking” sexual health. During his talk, Ramirez spoke about how educators might possibly shake things up, in some cases taking sex-ed outside the classroom in order to reach those who need it most. Happily, many educators are already doing this, systematically toppling many of the barriers that have long stood in their way. Throughout the conference, I was reminded of the many forms such resourcefulness can take. Here are the six most important fixes currently happening in American sex-ed.

1. Where can students get the answers they crave without fear of embarrassment or other negative repercussions? These days: their phones.

Sex-educators often employ anonymous question boxes in their classrooms, but the new-media generation is taking this idea of anonymity to the place where it thrives best: social media. I recently wrote about a variety of new social-media applications, YouTube series, and other online resources that allow teens to seek out accurate sexuality information anonymously. Since then, it seems that not a day goes by where I don’t hear about a new sex-ed app.

What’s important to remember about any of these sex-ed hacks is that just because a program works in one place, that doesn’t mean it will work in every community.

One of the more recent ones to catch my eye is Capptivation’s Reach Out, an app that provides sexual assault survivor resources to college-age students. According to Capptivation, a similar app for high schoolers is on its way. And the Healthy Teen Network — a membership-based advocacy organization  —  is in the process of developing two phone apps, one for high school-aged teens, and one for people who are older. They were inspired to do so after receiving an RFP (a request for proposal — a document from an agency soliciting a proposal for a specific commodity or service) from the Centers for Disease Control and Prevention (CDC). Alongside the United States Department of Health and Human Services, the CDC has been looking to fund the development of a mobile app that would support teen pregnancy prevention.

This push for sex-ed apps is not without precedent. A 2016 study on mobile phone-based interventions for smoking cessation showed that mobile interventions can lead to positive behavioral changes. And additional research  —  including a 2016 paper published in BMC Public Health  —  has shown that sexual-health apps remove certain barriers youth often feel in seeking out sexual-health services: namely, embarrassment. HTN is in the midst of conducting its own randomized control trials in order to determine the efficacy of its apps.

2. How can students take a leadership role in their own sex-education? Through peer-led sex-ed.

A recent review of 15 peer-led sexual-health education programs shows that peer-to-peer sex-ed can be successful at improving teens’ knowledge and attitude about sexual health  —  which is good news, considering that many teens don’t think adults are doing the best job. And just as with social-media apps, new peer-to-peer training programs are popping up all around the country. Teen PEP, which operates in both New Jersey and North Carolina, is one such program that trains teens to provide sex-ed to their peers at school. Another example is the team out of Planned Parenthood of North, Central, and South New Jersey, which leads an annual Teen Conference that students travel to on a one-day field trip.

In Austin, Texas, the Peer 2 Peer Project trains teens to teach both on school grounds and at other locations within their communities, going so far as to pay them for their efforts. In Baltimore, Maryland, the Healthy Teen Network and its subsidiary, the Healthy Teen Leadership Alliance, also empower teens to influence the field of sexual health. These are just a handful of programs among many that are handing the reins over to teens. It can be difficult to keep track of all the peer-led programs popping up around the country, but Advocates for Youth  —  an advocacy organization with its focus on adolescent sexual health  —  has gathered the results of numerous studies on the impact of peer education. These studies show how peer education reduces risky sexual behaviors and empowers teens, who seem to find their peers to be more credible than adult educators.

Inadequate sex education creating ‘health time bomb’

‘Shockingly high’ numbers of STI diagnoses prompt councils to call for compulsory sex education in UK secondary schools

[I]nadequate sex and relationships education (SRE) in schools is creating “a ticking sexual health time bomb”, councils are warning, amid concern over high numbers of sexually transmitted infections (STIs) among young people.

The Local Government Association (LGA), which represents 370 councils in England and Wales, has joined the growing clamour urging the government to make sex education compulsory in all secondary schools. Currently it is mandatory in local authority-maintained schools, but not in academies and free schools which make up 65% of secondaries.

Izzi Seccombe, chair of the LGA’s community wellbeing board, said it was a major health protection issue. “The lack of compulsory sex and relationship education in academies and free schools is storing up problems for later on in life, creating a ticking sexual health time bomb, as we are seeing in those who have recently left school.

“The shockingly high numbers of STI diagnoses in teenagers and young adults, particularly in the immediate post-school generation, is of huge concern to councils.

The LGA argues that it is a health protection issue, with 141,000 new STI diagnoses for 20- to 24-year-olds in England in 2015 and 78,000 for those aged 15-19. Sexual health is one of local government’s biggest areas of public health spending, with approximately £600m budgeted annually.

The LGA appeal came as the government was reported to be close to making an announcement regarding SRE and PSHE (personal, social, health and economic education), after the education secretary, Justine Greening, flagged up the issue as a priority for government.

Campaigners hope the announcement will be made during the next stage of the children and social work bill, which is passing through parliament. An amendment with cross-party support was tabled last week which, if carried, would would amount to the biggest overhaul in sex education in 17 years, but it is not yet clear what the government announcement will amount to, and crucially whether it will make SRE compulsory.

Seccombe said: “We believe that making sex and relationship education compulsory in all secondary schools, not just council-maintained ones, could make a real difference in reversing this trend, by preparing pupils for adulthood and enabling them to better take care of themselves and future partners.”

The LGA says while SRE should be made compulsory for secondary school children, with statutory guidance on key issues including sexual health, parents should still be given the option of taking their children from the lessons.

Tory MP Maria Miller was among those proposing the amendment to the bill last week. It followed an inquiry by the women and equalities committee, chaired by Miller, which heard that most children have seen online pornography by the time they leave primary school and two thirds will have been asked for a sexual digital image of themselves before they leave secondary school.

According to Miller, research has shown that just one in four children at secondary school receives any teaching on sex and relationship issues, and Ofsted has said that when it is taught the quality of teaching is often poor.

“Different interest groups cannot agree on a way forward that suits them and in the meantime we are letting down a generation of children who are not being taught how to keep themselves safe in an online, digital world,” said Miller.

“We are not teaching them that pornography isn’t representative of a typical relationship, that sexting images are illegal and could be distributed to child abuse websites and how to be aware of the signs of grooming for sexual exploitation.

“Overwhelmingly parents and children are fed up and want change. They want compulsory lessons in school to teach children and young people about consent and healthy relationships.”

What I learned about mental health from BDSM

[N]ot too long ago, I took my first step into the world of kink. I was a baby gay coming to terms with my borderline personality disorder (BDP) diagnosis, looking for any and every label that could help alleviate the lack of self-identity that comprises my BPD.

I knew I was queer. I knew I identified as femme. But I didn’t know if I was a dominant (top), a submissive (bottom), or a pillow princess; I didn’t even know if I was kinky.

So I tried to find out.

I began to notice a pattern. The sheer rush of euphoria and affection created a high I felt each time I “topped” my partner, and it would sharply drop the minute I got home. I was drained of energy and in a foul mood for days, often skipping work or class. I felt stuck on something because I wanted to feel that intensely blissful sex all over again, but I couldn’t figure out how to get it back.

If you’re familiar with the after-effects of taking MDMA—the crash, the lack of endorphins, the dip in mood for up to a week later—then you’ve got a pretty good idea of how a “drop” felt for me. Just add in an unhealthy serving of guilt and self-doubt, a pinch of worthlessness and a dash of contempt for both myself and my partner, and voila! Top drop: the less talked about counterpart to sub drop where the dominant feels a sense of hopelessness following BDSM—bondage and discipline, domination and submission, sadism and masochism—if after care is neglected.

In the BDSM community, it’s common to talk about the submissive (sub) experience: To communicate the expectations and needs of the submissive partner before engaging in consensual kinky play, to make sure the safety of the sub during intense physical and/or psychological activities is tantamount, to tend and care for the sub after the scene ends and they’re brought back down to earth.

Outside of this, the rush of sadness and anxiety that hits after sex is known as post-coital tristesse, or post-coital dysphoria (PCD). It is potentially linked to the fact that during sex, the amygdala—a part of the brain that processes fearful thoughts—decreases in activity. Researchers have theorized that the rebound of the amygdala after sex is what triggers fear and depression.

A 2015 study published in the Journal of Sexual Medicine found that 46 per cent of the 230 female participants reported experiencing PCD at least once after sex.

Aftercare is crucial and varies for subs, depending on their needs. Some subs appreciate being held or cuddled gently after a scene. Others need to hydrate, need their own space away from their partner or a detailed analysis of everything that happened for future knowledge. But no matter what the specific aftercare is, the goal is still the same: for a top to accommodate a sub and guide them out of “subspace”—a state of mind experienced by a submissive in a BDSM scenario—as directly as they were guided in.

I asked one of my exes, who’s identified as a straight-edge sub for several years, what subspace is like. As someone who doesn’t drink or do drugs, I was curious about what it was like for them to reach that same ephemeral zone of pleasure.

“It gets me to forget pain or worries, it gets me to focus only on what I’m feeling right then,” they told me. “It’s better than drugs.”

My ex gave up all substances in favour of getting fucked by kink, instead. I’m a little impressed by how powerful the bottom high must be for them.

“The high for bottoms is from letting go of all control,” they added. If we’re following that logic, then the top high is all about taking control.

We ended the call on a mildly uncomfortable note, both trying not to remember the dynamics of control that ended our relationship.  Those dynamics were created, in part, by my BPD, and, as I would later discover, top drop.

In the days to follow, I avoided thinking about what being a top had felt like for me and scheduled a lunch date with another friend to hear his perspective.

“Being a dom gives you the freedom to act on repressed desires,” he told me over a plate of chili cheese fries. This is what his ex said to cajole him into being a top—the implied “whatever you want” dangled in front of a young gay man still figuring himself out.

He was new to kink, new to identifying and acting on his desires, and most of all, new to the expectations that were placed on him by his partner. He was expected to be a tough, macho top to his ex’s tender, needy bottom. His after-care, however, didn’t fit into that fantasy. If that had been different, maybe he wouldn’t have spiraled into a place where his mental health was deteriorating, along with his relationship.

The doubt and guilt that he would often feel for days after a kinky session mirrored my own. We both struggled with the idea that the things our partners wanted us to do to them—the things that we enjoyed doing to them—were fucked up. It was hard to reconcile the good people that we thought we were, the ones who follow societal expectations and have a moral compass and know right from wrong, with the people who are capable of hurting other people, and enjoying it.

For my friend, there was always a creeping fear at the back of his mind that the violence or cruelty he was letting loose during sex could rear up in his normal life, outside of a scene.

For me, there was a deep instinct to disengage, to distance myself emotionally from my partner, because I thought that if I didn’t care about them as much, then maybe I wouldn’t hate them for egging me on to do things I was scared of.

My friend has since recognized how unhealthy his relationship with his ex was. These days, he identifies as a switch (someone who alternates between dominant and submissive roles). The deep-seated sense of feeling silenced that was so prevalent in his first kinky relationship, is nowhere to be seen. He communicates his sexual needs and desires and any accompanying emotional fragility with his current partner. He’s happy.

I’m a little envious of him. My second-favourite hobby is rambling about all of the things I’m feeling, and it’s a close second to my favourite, which is crying. I credit my Cancer sun sign for my ability to embrace my insecurities, but there’s still something that makes me feel like I’m not equipped to deal with top drop.

There’s an interesting contrast between how a top is expected to behave—strong, tough, in control—and the realities of the human experience. When a top revels in the high of taking control, but starts to feel some of that control fading afterwards, how do they pinpoint the cause? How do they talk about that insecurity? How do they develop aftercare for themselves?

Why are some women never able to orgasm? A gynaecologist explains

Despite modern society being able to openly discuss female sexuality, there remains a number of existing taboos.

One of the most glaring is female orgasms. Women are rarely taught about the intricate details of their anatomy and often work these things out through their own experimenting.

What is the best way to get an orgasm? How often should I have one? Should I be able to have one during penetrative intercourse? Why have I never had one? – questions not uncommon to hear among small friendship groups of women over a bottle of wine.

Dr Sherry A Ross, an LA-based gynaecologist with 25 years experience aims to educate with a complete guide to the vagina in her new book She-ology: The Definitive Guide to Women’s Intimate Health. Period.

In the foreword of her book, Dr Sherry notes that “talking about the mighty V outside of doctor’s offices and bedrooms has remained a major taboo” and devoted an entire chapter to the female orgasm. The Independent asked the gynaecologist and obstetrician all the questions about female orgasms that are rarely spoken about.

Why might some women never orgasm?

Attitudes regarding sex, sexuality and gender vary greatly between different cultures and religions. Certain sexual practices, traditions and taboos are passed down through generations, leaving little to the cause of female pleasure or imagination.

For some women, finding and/or enjoying sexual intimacy and sex is difficult, if not impossible. Research suggests that 43% of women report some degree of difficulty and 12% attribute their sexual difficulties to personal distress. Unfortunately, sexual problems worsen with age, peaking in women 45 to 64.  For many of these women the problems of sexual dysfunction are treatable, which is why it is so important for women to share their feelings and concerns with a health care provider.

Unfortunately, there has been a history of “gender injustice” in the bedroom. Women have long been ignored when it comes to finding solutions to sexual dysfunction. In short, there are twenty-six approved medications for male erectile dysfunction and zero for women. Clearly, little attention has been paid to the sexual concerns of women, other than those concerns that involve procreation.

How many women might never orgasm?

During my 25 years in private practice, I’ve met a number of women in their 30s, 40s and 50s who have never even had an orgasm. In fact, 10 to 20% of all women have never experienced one.

Issues related to sex are not talked about enough even with a health care provider. Let’s just start by saying, 65 per cent of women are embarrassed to say the word vagina and 45 per cent of women never talk about their vagina with anyone, not even with their doctor.

Some patients say they have pain with sex, have problems with lubrication, don’t have a sex drive or don’t enjoy sex.  My first question is “Are you having problems in your relationship?”, “Do you like you partner?” , “Are you able to have an orgasm?”, “ Do you masturbate?” These open-ended questions tend to bring out sexual dysfunction including the inability to have an orgasm.

There is a great deal of embarrassment and shame when a woman admits she has never experienced an orgasm.

Is the inability to not orgasm normal?

The inability not to have had an orgasm can reflect women’s inability to know they own anatomy and may not be a disorder at all. In a survey of women aged 16-25, half could not find the vagina on a medical diagram. A test group of university- aged women didn’t fare much better with one third being unable to find the clitoris on a diagram. Clearly, if you can’t find it, how are you going to seek enjoyment from it?

Women must first understand what brings them pleasure and in their pursuit of happiness they have to understand where their clitoris is and how to stimulate it. Masturbation is a skill.  It has to be learned, just as walking, running, singing and brushing your teeth.

What is an orgasm disorder and how would you categorise one? 

The inability to have an orgasm falls under the category of Female Sexual Dysfunction of which there are five main problems: low libido or hypoactive sexual desire disorder, painful sex, sexual arousal disorder, an aversion to sex and the inability to orgasm.

Hypoactive sexual disorder, the most common female sexual dysfunction, is characterised by a complete absence of sexual desire. For the 16 million women who suffer from this, the factors involved may vary since sexual desire in women is much more complicated than it is for men. Unlike men, women’s sexual desire, excitement and energy tend to begin in that great organ above the shoulders, rather than the one below the waist. The daily stresses of work, money, children, relationships and diminished energy are common issues contributing to low libido in women. Other causes may be depression, anxiety, lack of privacy, medication side effects, medical conditions such as endometriosis or arthritis, menopausal symptoms or a history of physical or sexual abuse.

You are the person in charge of your vagina and clitoris. First and foremost, get to know your female parts intimately. Understanding your sexual response is a necessary health and wellness skill. Make mastery of that skill a priority.

Childhood cancer treatment may hinder later-life sexual relationships

 

[N]eurotoxic treatment for cancer during childhood may influence sexual activity and relationships in adulthood, according to new research.

Study co-author Vicky Lehmann, Ph.D., of Nationwide Children’s Hospital and Ohio State University – both in Columbus, OH – and colleagues found that adults who received high-intensity neurotoxic treatment for cancer as a child were less likely to meet certain sexual and romantic milestones.

However, the team found that childhood cancer treatment did not affect overall satisfaction for sexual and romantic relationships in adulthood.

Lehmann and team recently reported their findings in the journal Cancer.

According to the American Cancer Society, it is estimated that around 10,380 children aged 15 and under were diagnosed with cancer in the United States last year.

Leukemia is the most common form of childhood cancer, accounting for around 30 percent of all cases, followed by brain and spinal cord tumors, which make up around 26 percent of all childhood cancer cases.

Childhood cancer treatment and psychosexual development

While cancer was responsible for more than 1,200 childhood deaths last year, over 80 percent of children diagnosed with the disease will survive for at least 5 years. This is due to significant advances in cancer treatment, which include surgery, chemotherapy, and radiation therapy.

However, such treatment is certainly not without risk. For example, studies have shown that cranial radiation – often used to treat brain tumors – may cause harm to the developing brain, leading to long-term neurocognitive impairment.

Previous research has shown that neurocognitive impairment as a result of childhood cancer treatment may impact social interaction in adulthood, but studies investigating the effects of such treatment on psychosexual development are few and far between.

“Psychosexual development entails reaching certain milestones, such as sexual debut, entering committed relationships, or having children.

It is a normative part of becoming an adolescent or young adult, but only comparing such milestones without taking satisfaction into account falls short. These issues are understudied among survivors of childhood cancer.”

To address this gap in research, the team enrolled 144 survivors of childhood cancer aged between 20 and 40. A further 144 participants who were not treated for childhood cancer (the controls) were matched by age and sex.

All participants completed questionnaires on psychosexual development, sexual satisfaction, and relationship satisfaction.

To determine the brain toxicity of cancer treatments in childhood, the researchers used data from the participants’ medical charts.

Neurotoxic cancer treatment might predict later-life psychosexual issues

Overall, the team found that adults who were treated for cancer in childhood did not differ significantly from the controls in terms of psychosexual development, sexual satisfaction, and relationship satisfaction.

However, on analyzing subgroups of childhood cancer survivors, the researchers found that those who previously received treatments high in neurotoxicity were less likely to have had sexual intercourse, be in a relationship, or have had children, compared with controls.

The type of cancer treatment in childhood did not appear to affect sexual satisfaction, the team reports. “This highlights the subjective nature of psychosexual issues, and the importance of addressing any concerns in survivorship care,” notes Lehmann.

The researchers say that their findings indicate that the neurotoxicity of cancer treatment in childhood may predict the likelihood of psychosexual problems in adulthood. They add:

“Additional research is needed to delineate how neurocognitive impairment undermines social outcomes for survivors, as well as other related factors.

Given the findings of the current study, healthcare providers should assess romantic/sexual problems among survivors, especially those who received high-dose neurotoxic treatments. Referrals to psychosocial care could prevent or reduce potential difficulties.”

Have you ever had ‘unjust sex’?

Unthinkable: Examples include ‘women being pressured – not quite to the point of outright coercion – to have sex, or to have sex without contraception’, says philosopher Ann Cahill

“We need to remember that sexual assault is not the only kind of sexual interaction that is ethically problematic,” says author Ann Cahill.

[U]ncertainty surrounding the boundaries of ethical sexual activity is not confined to boozed-up young adults or American presidents. Among academics there is discussion about what distinguishes rape and sexual assault from another category of “ethically problematic” sex.

Examples of “unjust sex” include “women being pressured – not quite to the point of outright coercion, but pressured uncomfortably nonetheless – to have sex, or to have sex without contraception,” explains Ann Cahill, author of a number of books on gender issues including Rethinking Rape.

Cahill, professor of philosophy at Elon University in North Carolina who is visiting Dublin this week, says she has tried to “figure out in more detail” what distinguishes sexual assault from “unjust sex”, drawing on the work of New Zealand psychologist Nicola Gavey.

Her analysis has led her to challenge the traditional feminist concern with “objectification”: treating women’s bodies as objects. Instead, she uses “derivatisation” – treating women as “stunted persons, persons whose identity and behaviour is primarily or entirely limited by the desires of another person” – as a standard by which to measure actions.

Cahill says “we need to remember that sexual assault is not the only kind of sexual interaction that is ethically problematic. Too often our approach to sexual ethics is limited by relying solely on the presence of consent, a reliance that obscures other crucial elements in sexual interactions that are ethically relevant”.

How do you distinguish “unjust sex” from rape?

“Briefly, I argue that examples of unjust sex and incidents of sexual assault share an indifference to women’s sexual preferences, desires and wellbeing, and that’s what explains how unjust sex perpetuates and upholds rape culture. In both cases, the specific sexuality of the woman is not participating robustly in the creation of the sexual interaction.

“What distinguishes the two examples, I then argue, is the specific role that the woman’s sexual subjectivity plays. In the case of examples within the grey area of unjust sex, women’s agency plays an important role: if a man repeats a request for or invitation to sex multiple times, for example, that very repetition indicates that the woman’s consent is important.

“However, I also argue that the role that the woman’s agency plays is a problematically stunted one that limits the kind of influence she can have on the quality of the interaction that ensues, and does so to such an extent that it renders the interaction unethical.

“In the case of sexual assault, the woman’s agency is either overcome – by force, or coercion, or other methods – or undone entirely, by use of drugs or alcohol.”

Where does “objectification” come into this, and does sexual attraction always entail some element of it?

“Feminists have long used the notion of objectification as an ethical lens, and specifically, as an ethically pejorative term. And certainly I do think that many of the social and political phenomena that feminists have criticised by using the term ‘objectification’ – dominant forms of pornography, oppressive medical practices, common representations of women’s bodies – are worthy of ethical critique.

“However, I worry about what the term ‘objectification’ implies, and when I dug into the philosophical literature that sought to really unpack the term, my worries only intensified. If objectification means, roughly, to be treated as a thing – a material entity – and if it is virtually always ethically problematic, then it seems we are committed to a metaphysics that places our materiality in opposition to our humanity or moral worth.

“But what if our materiality, our embodiment, is not contrary to our humanity or moral worth, but an essential part of it? If we approach embodiment in this way, then to be treated like a thing is not necessarily degrading or dehumanising. In fact, having one’s body be the object of a sexualising gaze and/or touch could be deeply affirming.

“Getting back to your question: does sexual attraction require objectification? The short answer is yes: sexual attraction requires treating another body as a material entity. But that does not mean that sexual attraction is necessarily ethically problematic.”

You say women “are encouraged, and in some cases required, to take on identities that are reducible to male heterosexual desires”. How do women avoid being so “derivatised” while in a relationship?

“This is a tricky matter, because human beings are intersubjective.

“Equal and just relationships among individuals require the recognition that they have a substantial contribution to make to those relationships, and that no relationship should position one of the individuals involved in it as the raison d’être of the relationship itself.”

Is the power dynamic always working in one direction, however? Women are capable of objectifying men. Should that concern us too?

“As I state above, objectification is not necessarily ethically problematic. And so to the extent that women have the capacity to treat men’s bodies as material entities, yes, they can objectify them.

“However, in our current political and social situation, women’s objectification of men’s bodies is far less common than men’s objectification of women’s bodies; even more importantly, it rarely amounts to derivatisation and does not serve to undermine men’s political, social, and economic equality.

“When I say that it does not amount to derivatisation, I mean that heterosexual men are less likely to view their bodies solely or persistently through the lens of how they appear to heterosexual women, and they rarely see male bodies represented in dominant media as defined primarily or solely through how those bodies appear to heterosexual women.

“While it’s not impossible for women to derivatise men – one can imagine, for example, a woman evaluating a man as a sexual partner solely on the basis of whether he matches her sexual preferences – structurally, those examples of derivatisation don’t add up to the kind of persistent inequality that still tracks along gender lines.

“For example, as political candidates, men don’t suffer for failing to meet the aesthetic ideals of heterosexual women, while women do suffer for failing to meet the aesthetic ideals of heterosexual men. Of course, they also suffer for meeting those ideals too well, because feminine beauty, while allegedly admirable in women, is also associated with shallowness and lack of intellect.

“Although I haven’t written about this before, however, it seems to me that hegemonic masculinity does have a derivatising effect on heterosexual men, to the extent that it requires them to derivatise women. In this sense, the subjectivity of heterosexual men is stunted to the extent that it is required to engage in the kinds of behaviour that demonstrates disrespect of women as moral equals – behaviour that is necessary for other heterosexual male subjects to be confirmed or affirmed in their own forms of masculinity.

“To the extent that heterosexual men can find their standing within homosocial relations threatened or troubled if they refuse to derivatise women, or at least pretend to, then they are also subject to a failure to recognise their own ontological distinctness.”

Should Shame Be Used to Treat Sexual Compulsions?

The concept of “sex addiction” has become deeply embedded in our culture — people toss the term around pretty easily, and it’s the subject of TV shows, documentaries, and a profitable cottage industry of treatment centers. The problem is, as Science of Us has noted before, the scientific evidence for sex addiction being similar to alcohol or drug addiction is very, very thin, and it may be the case that people who believe or are told they have sex addiction actually have other stuff going on.

And yet, it’s undoubtedly the case that many people show up at therapists’ offices worried about sexual behavior that feels compulsive. How do therapists who are skeptical of the idea of sex addiction deal with these patients? That’s the question at the center of an interesting article in SELF by Zahra Barnes.

Barnes does a good job laying out the strong majority view that “sex addiction” shouldn’t be viewed in the same way as other, more scientifically validated forms of addiction, and she also contrasts the way different sorts of therapists deal with sexually compulsive behavior. As she explains, therapists who hew to the majority view often take a “harm reduction” approach to patients who are complaining of compulsive behavior.

“It’s humanistic, meaning it privileges the subjective experience of a person and doesn’t try to apply some external model on what they’re describing, and it’s culturally libertarian, meaning as long as they’re not hurting anyone, you allow people to behave the way that they want and give them the space to do it,” said Michael Aaron, Ph.D., a sex therapist in New York City and author of Modern Sexuality.]

This method can work for people troubled by their sexual urges and those with compulsive sexual behavior. “Rather than trying to change something, we need to acknowledge it and embrace it,” Aaron says. He offers the example of someone who has fantasies of traumatizing children sexually or being sexually violent toward women: “The harm reduction approach asks, can you play out some of these themes with a consenting partner?” The aim is to satisfy these desires with a willing partner instead of suppressing them, which can just make them stronger, he explains.

Therapists who do believe in the addiction model work differently, and where this difference manifests itself most strongly is in their approach to shame. While Aaron and other harm-reduction researchers try to stay away from shaming their patients, which they say can worsen compulsive behaviors, believers in the sex-addiction model see things differently:

“Sex addicts need to feel some shame about what they’re doing, because they are shameless. When people are shameless, they rape and murder and steal and pillage and get into politics,” [says Alexandra Katehakis, clinical director of the Center for Healthy Sex.]. But this is different from shaming someone, she says. “Shaming in an unprincipled way is out of bounds [for a mental health professional],” she explains. That would include saying or even implying that someone is disgusting based on what they’re doing. Rather, she asks questions designed to make someone reflect on what their actions have wrought, like, “What do you think that feels like for your partner?” It’s helpful, not damaging, she explains, because, “It challenges them to see what they’re doing, and it brings them into the reality of their behavior.”

It seems like one of the key philosophical differences here is the question of the extent to which people can control their most primal sexual urges. The therapists who don’t believe in sex addiction appear to view people’s sexual preferences (for lack of a better term given they probably aren’t preferences) in a holistic context — if people are “acting out” sexually in a way that harms others, it could be because of other stuff going on in their lives. You address the behavior by addressing the root causes. The believers, on the other hand, focus more on the urges and finding ways to address the behavior and urges in and of themselves.

These approaches aren’t fully compatible, so it’s no surprise there’s tension between the majority of sex researchers who don’t believe in the addiction model and the minority who do.

What is an orgasm?

The female orgasm — much like the male orgasm — at its very base is a physical, pleasurable reflex when the woman’s genitals relax during sex. During intercourse, the muscles in the body are tightened, and when the female orgasm occurs, they release and return to what is known as the pre-arousal stage.

Depending on a woman’s anatomy and unique being, she may be able to have multiple orgasms in a row. What you may find is that some women may feel embarrassed or ashamed to let go and be turned on — or they may think they are taking too long to achieve an orgasm and believe that they are being a burden to you.

Still others may find it challenging to have an orgasm because anatomically, their clitoris is too far away from their vagina. Researchers have discovered that typically, if your clitoris is more than 2.5 centimeters away from your vagina, or roughly the tip of your thumb to your knuckle, that you may not be able to achieve an orgasm by penile penetration alone. That doesn’t mean they can’t achieve orgasm through intercourse. It just means you need to work a little harder and be little more creative to find what really turns on your partner.

A very low percentage of women — less than 10 percent — claim that they can achieve an orgasm by penile penetration alone. It’s more likely that your partner prefers and needs more than one method of stimulation. So from oral sex to masturbation to using a vibrator — there are many different ways you can get your female partner to reach climax. It’s just a matter of knowing her anatomy and what she prefers in bed.

Overall, however, it’s really important that you create a safe and welcoming environment for your woman to relax and really let go. In that trusted space, she will be able to open up to you and tell you what she wants — what she wants you to say, how she wants you to touch her and what her fantasies are. Create an Environment for Intimacy

You’ll want to start out the night by creating a safe, trusted and intimate environment that will make your woman feel comfortable and loved. Women like many different environments for sex, and again, no one woman is alike.

So you need to know your woman well. Does she respond to flowers, candles and romance? Does she want sex quick and dirty? Does she need a chance to unwind with a glass of wine or a hot bath? Whatever her triggers are for relaxation and comfort, you’ll want to deploy those for her.

What this does is let her know you are watching, listening and responding to what will make her feel most wanted and loved. So pay attention — or ask her — and that will go a long way in creating a better environment for being vulnerable when it comes time to making that climb toward the female orgasm.

Kissing is Key

If you want to give a woman an orgasm, kissing is going to be key. Lower yourself to her vagina and use your tongue to massage her clitoris with slow licks. Pay attention to her breathing as you are doing this, as you may want to speed up or slow down depending on how she is responding.

Some patterns think that if they do everything quickly, then that is a turn on. But that’s likely going to make her feel like she needs to perform and fake an orgasm because she knows it’s not going to come quickly.

Instead, ask her what is feeling good as you are doing it. Ask her if she wants more kissing, more tongue licking or flicking, or the speed to be faster or slower. If she feels comfortable with you, she will tell you what is feeling especially good.

Ask her to guide your head as you are giving her oral sex so that you know the exact position that feels the best.

A bonus move that works really well: Ask her to masturbate if she feels comfortable while you are kissing or licking her, as you can watch her do this and pay attention to where her fingers are going. She is going to know her body the best, and you can know the exact location of where your tongue or fingers should be next.

Start Out Slowly When Penetrating

Another urban myth about penetrating a woman with your fingers, also called “fingering.” You can’t do it quickly at first. If you’ll remember from the first section, a woman’s muscles are usually tight during sex. When she orgasm’s they contract.

Leading up to the Big O, her muscles will begin to relax and it will be easier to penetrate her and arouse her as you lead her to an orgasm. But at the beginning, start out slowly.

Use your mouth to apply a good amount of saliva to her vagina so that your fingers can slip in fairly easily. Start with one finger and move it very slowly back and forth. If you find that there is more room and that she is getting more aroused with one finger, try to insert two fingers.

Move those two fingers back and forth very slowly, while asking your partner if she is enjoying it along the way. If she is showing signs of discomfort or pain, stop. Communication is really key as you are participating in fingering because your woman will give you clues that she is ready for penetration with your penis.

If she prefers fingering over your penis, then continue in the method of moving your fingers in and out slowly. When she is just out of breath and close to having an orgasm pull out your fingers and begin using your tongue to rapidly flick her clitoris. Continue massaging the area around the clitoris as you are flicking it until she reaches orgasm and screams or sighs in delight.

You may not get verbal affirmation as not every woman is not a screamer. But, ask her if she is reaching orgasm and pay attention to her body. Usually a woman will become very sensitive and she won’t be able to handle you touching her in her vaginal region any longer. She’ll need some time to reset. Some women can have an other orgasm a few minutes later. Keep that communication open so you know what to expect and exactly what you need to do to get her to that place of absolute pleasure.

Should I Be Ashamed of Using a Vibrator?

We get this question a lot — and the answer is you absolutely should be willing to use a vibrator. It says nothing about you that your female partner is not achieving orgasm with your penis alone. It’s actually quite common that this happens because sex takes a lot of practice to get both partners to achieve that pleasurable moment.

So if this is the challenge that you are experiencing — or even if you’re not — try a vibrator! They are fun and safe to use. They come in a wide variety of sizes and textures so that you can experience different sensations. This is especially a great way for a woman who hasn’t been extremely communicative about what she likes sexually to experiment with and decide what she truly loves — and wants you to try to replicate!

Remember to Engage Your Brain

The ability to reach an orgasm is more than half of your brain. You have to exert mental energy to reach that level of being able to let go. If you’ve been able to do it, then it’s good to encourage your partner that it can happen for her as well.

Before you engage in any kind of sexual activity, sit down with your partner and talk to her about expectations and what she should expect out of you. Let her know that you are there for her — to pleasure her and to make her feel good. That’s going to put her at immediate ease and let her know that you are there for her. You’re not there to get the first orgasm. You want her to be happy first.

[A]dvocates and researchers collaborated to create more than 18 3D figures that model sex organs during a various states of arousal. They range from a flaccid penis to a dilated vaginal opening, allowing students to “feel” their way though sexual health lessons.

While it may be a NSFW (let alone not-safe-for-school) endeavor, these models are game-changers for blind students who often need to learn about sexual health through verbal instruction alone.

Sex ed classes overall often rely on dull videos and static illustrations, and while that type of stale education is a disservice to all students, it presents a unique problem for blind students.

“That approach does a blind student no good whatsoever because they, of course, cannot see the pictures and videos.” Dr. Gaylen Kapperman, a professor at Northern Illinois University who was involved with the project, told Mashable via email.

Studies show that 61% of blind adults or those with low vision say their vision status had a negative impact on the way they were able to participate in sex education.

It’s a gap advocates and researchers at Benetech, a nonprofit organization specializing in tech for good, set out to solve by creating these models of various penises and vulvas.

“3D models are the only types of models that make any sense to blind people,” Kapperman said. “Many people believe that if you provide raised-lined 2D tactile pictures of sex organs that blind people will be able to generalize this information. [That approach] makes no sense whatsoever for blind persons.”

But these models don’t only break sex ed barriers for blind children. Researchers say the models could make the instruction more meaningful for sighted kids, too.

The project’s goal is to eventually provide open-source 3D printing files for teachers. This means school districts would only have to finance the materials and printers to make the models.

Many experts predict the technology will become a staple for schools anyway. Once a school district owns a printer, 3D printing is a low-cost way to create models for classroom instruction, making it ideal for schools on a budget.

A sizable 90% of blind students attend school with sighted children, relying on modified lessons to fully absorb material. But there are only about 61,700 blind students in the U.S. Buying commercial models of genitalia already on the market can cost up to $500 per model — something low-funded schools would likely be reluctant to do, especially when only a handful of blind students may ever pass through their district.

To develop prototypes, Benetech partnered with LightHouse for the Blind and Northern Illinois University, where the models were first tested on blind college students. The project was funded entirely by a private Benetech donor.

Now in the second phase of the pilot program this spring, the models will make their ways into the hands of middle school and high school students — the target demographic.

By the end of the 2017 school year, researchers hope to have feedback from students on the current prototypes. Then they’ll release files with detailed printing instructions for classroom use.

Benetech plans to offer pre-printed models to accommodate schools without 3D printers, for a fee much lower than commercial models.

“It is our hope that these models will be an effective teaching tool for teachers to communicate sex education in a way that works for students who are blind and visually impaired,” said Dr. Lisa Wadors Verne, program manager of education and partnerships at Benetech.

Talking With Both Daughters and Sons About Sex

Parents play a key role in shaping sexual decision-making among adolescents — especially for girls.

A 2016 review of more than three decades of research found that teenagers who communicated with their parents about sex used safer sexual practices. Likewise, new research from Dutch investigators who studied nearly 3,000 teenagers found that young adolescents who reported feeling close with a parent were unlikely to have had sex when surveyed again two years later.

Notably, both research teams found that daughters benefited more than sons, and that the effective conversations and relationships were typically had with mothers.

According to Laura Widman, lead author of the review study and an assistant professor of psychology at North Carolina State University, “parents tend to talk about sex more with daughters than with sons, and we can speculate that that’s what’s probably driving these findings. Boys may not get the messages as frequently or have the kind of in-depth conversations that parents are having with girls.”

Given the results of her research, Dr. Widman said that she “wouldn’t want parents to get the idea that they only need to talk to daughters. In fact, it may be the opposite. We need to find a way to help parents do a better job of communicating with both their sons and daughters so that all teens are making safer sexual decisions.”

That parents have more frequent conversations with their daughters about sex and sexual development may be prompted by biological realities. Menstruation, HPV vaccination (which remains more common in girls than boys), and the fact that birth control pills require a prescription might spur discussions that aren’t being had with sons.

Yet experts also agree that gender stereotypes play a powerful role in sidelining both fathers and sons when it comes to conversations about emotional and physical intimacy. Andrew Smiler, a psychologist who specializes in male sexual development, noted that women generally “have a better vocabulary for talking about feelings and relationships than boys and men do. Fathers may be a little more stoic, more reserved and more hands-off.” And, he added, “they may play to the stereotype of trusting boys to be independent and able to care for themselves.”

These same stereotypes can also tend to steer the conversation in one direction with daughters and another direction with sons. When parents do address sexual topics with their teenagers, they typically adopt a heterosexual frame with boys playing offense and girls playing defense.

“We usually view our girls as potential victims who need to be protected from pregnancy and rape,” says Sheryl Ziegler, a psychologist who provides mother-daughter seminars on puberty and sexual development, while boys are often cast as testosterone-fueled prowlers looking for nothing but sex. These assumptions often drive how parents approach the conversation. Dr. Mary Ott, an associate professor of pediatrics at Indiana University and the author of a research synopsis on sexual development in adolescent boys observed that, “when parents talk with boys, there’s an assumption that they’ll have sex and they are advised to use condoms. Whereas for girls, there’s more of a focus on abstinence and delaying sex.”

Parental concern about the negative consequences of adolescent sexual activity can reduce “the talk” to a laundry list of don’ts. Don’t get a sexually transmitted infection, don’t get pregnant or get a girl pregnant and don’t proceed without gaining consent. Critical as these topics are, Dr. Ziegler points out that they can “become the focus, so much more than having a quality conversation about why we are sexual beings, or talking about all of the ways we can express love.” And failing to acknowledge the pleasurable side of sex can, according to Dr. Smiler, hurt the credibility of adults. “When parents only acknowledge the scary side of the story,” he said, “teenagers can devalue everything else the parents have to say.”

So how might we do justice to conversations with both our daughters and sons about emotional and physical intimacy?

Over the years in my work as a clinician, I’ve come to a single tack that I take with adolescent girls and boys alike. First, I prompt teenagers to reflect on what they want out of the sexual side of their romantic life, whenever it begins. Why are they being physically intimate, what would they like to have happen, what would feel good?

Following that, I encourage each teenager to learn about what his or her partner wants. I urge them to secure not just consent, but enthusiastic agreement. Given that we also grant consent for root canals, gaining mere permission seems, to me, an awfully low bar for what should be the joys of physical sexuality. Dr. Smiler adds that any conversation about consent should avoid gender stereotypes and address the fact that boys experience sexual coercion and assault and “include the idea that boys can and do say no.”

Finally, if the parties are enthusiastically agreeing to sexual activity that comes with risks — pregnancy, infection, the potential for heartbreak, and so on — they need to work together to address those hazards.

Research suggests that this shouldn’t be a single sit-down. The more charged the topic, the better it is served, and digested, in small bites.

Further, returning to the topic over time allows parents to account for the rapidly shifting landscape of adolescent sexual activity. We should probably be having one conversation with a 12-year-old, an age when intercourse is rare, and a different one with a 17-year-old, half of whose peers have had sex.

Is it better for mom or dad to handle these discussions? Teenagers “want to have the conversation with someone they trust and respect and who will show respect back to the teen,” Dr. Smiler said. “Those issues are more important than the sex of the person having the conversation.”

How families talk with teenagers about their developing sexuality will reflect the parents’ values and experiences but, Dr. Ott notes, we’re all in the business of raising sexually healthy adults.

“We want our teenagers to develop meaningful relationships and we want them to experience intimacy,” she said, “so we need to move our conversations about sex away from sex as a risk factor category and toward sex as part of healthy development.” And we need to do so with our sons as well as our daughters.

For Veterans, Trauma Of War Can Persist In Struggles With Sexual Intimacy

[M]uch has been said about the physical and psychological injuries of war, like traumatic brain injury or post-traumatic stress disorder. But what we talk about less is how these conditions affect the sexual relationships of service members after they return from combat.

Since 2000, service members who were deployed received at least 138,000 diagnoses of PTSD. More than 350,000 have been diagnosed with traumatic brain injury since 2000. Evidence suggests the numbers are actually higher because many don’t seek treatment.

These conditions cause their own sexual side effects, such as emotional numbness, loss of libido and erectile dysfunction. And the long list of medications used to treat PTSD, TBI and other medical conditions can worsen those side effects.

‘He would sleep for days’

Chuck and Liz Rotenberry of Baltimore struggled with their own challenges when Chuck returned from Afghanistan in 2011. He’s a former Marine gunnery sergeant who trained military working dogs. He left active duty in 2012.

For Liz and Chuck, sex had never been a problem. They’ve been married for 14 years and they’re still very much in love. Liz says she fell for Chuck in high school. He was that guy who could always make her laugh, who always had a one-liner ready and never seemed sad.

But when Chuck returned from Afghanistan, their relationship would soon face its greatest challenge. Baby No. 4 was just two weeks away; for sure, it was a chaotic time. But Liz noticed pretty quickly, something was terribly wrong with her husband.

“I wouldn’t be able to find him in the house and he wouldn’t be outside, and I’d find him in a separate bedroom just crying,” Liz says. “He would sleep for days. He would have a hoodie on and be just tucked away in the bed, and he wouldn’t be able to get out of bed. He would have migraines that were so debilitating that it kept him in the bed.”

When Chuck was in Afghanistan, an IED — improvised explosive device — exploded 3 feet behind him. Shrapnel lodged into his neck and back.

It would take three years for someone at the Department of Veterans Affairs to explicitly lay out for Liz that Chuck had developed severe post-traumatic stress and suffered a traumatic brain injury — and that she would need to be his caregiver.

The Marine self-image

During that three-year period, there were times Chuck estimates he was taking 15 to 16 different medications twice a day.

Sex was usually the furthest thing from his mind.

“I didn’t think about it. I wanted to be with Liz, I wanted to be near her,” he says. “When the desire was there, it was unique. It was rare, as opposed to the way it was before. And a lot of times, with the mountains of medication I was on, you know, in my head [it was] all systems go, but that message didn’t go anywhere else.”

Liz noticed that Chuck stopped initiating physical affection.

“The thought of him reaching out to me to give me a hug wasn’t existent. It was like I had to give him the hug. I now had to step in and show him love,” she says.

Sometimes months would go by before they would have sex.

“It started off as being pretty embarrassing, pretty emasculating,” Chuck says. “It was like, ‘Really? This too doesn’t work?’ You blame it on, ‘Oh, it’s just the medication,’ or ‘You’re tired,’ or whatever initially, and you don’t realize it’s stress or my brain just doesn’t work like it used to.”

Liz and Chuck had never really talked about sex in any serious way before. So they kept avoiding the conversation — until this year. That’s when Chuck finally asked his primary care provider for help. The doctor prescribed four doses of Viagra a month. Liz and Chuck say the medication has improved things substantially — though they joke about how few doses the VA allots them every month.

But asking for just those four doses took Chuck three or four visits to the doctor before he could work up the nerve. He says it can be especially hard for a Marine to admit he’s having problems with sex because it contradicts a self-image so many Marines have.

“You know, as a Marine, you can do anything. You believe you can do anything, you’ve been trained to do nearly anything,” he says. “You’re physically fit. You’re mentally sound. Those are just the basics about being a Marine.”

If he has any advice for a Marine going through the same thing he and his wife are facing, he says you need to talk about it. Bring it up with your spouse. Bring it up with your doctors.

“Marines always jokingly hand out straws. You got to suck it up. You got to do what you need to do to get it done,” Chuck says. “It’s just a different mission. … Don’t let your pride ruin what you worked so hard for.”

Caught in the modesty bind: Why women feel shy to consult doctors for their sexual well-being

“I was 17, when I first got sexually intimate with my boyfriend,” says Kriya (name changed), a 23-year-old IT professional from Hyderabad, while speaking to The News Minute.

“Later we were very scared, as it was the first time for both of us,” she recalls. She missed her periods that month. The 17-year old who had never once been to hospital alone, was scared and unsure of what to do next.

Trying to glean more information online just added to her worry over getting pregnant. Finally she discussed the issue with her boyfriend, and both of them decided to consult a gynaecologist.

“I was already very scared. After I told the receptionist my age, she kept staring at me. It made me so uncomfortable. While other patients were called by name, when it was my turn, she said ‘Aey, hello.…go!’ I felt so bad.

I expected at least the doctor to act sensitive. She first asked me what happened. When I told her, she started lecturing to me about our culture, and how young I am. It was a horrible experience. After the check-up, once I reached home, I burst out crying,” she shares.

From then on, Kriya has always felt too scared to discuss any sexual health problem with a gynaecologist. She is now 23, but in her view, nothing much has changed.

“Last month, I had rashes all over my vagina right up to my thigh. I just could not walk. It was painful. In the beginning, I used anti-allergic medication and antiseptic cream. But I was finally forced to go to a doctor. But even this time, I was ill-prepared for those weird looks.

The receptionist first asked for my name, then my husband’s name. For a moment, I panicked. After a pause I said, I am unmarried.”

Kriya feels that such unnecessary queries have nothing to do with a particular health problem and should not be asked: “We are adults and should not be judged for such things. After all, it is my decision. But society does not think so.”

Dr Kalpana Sringra, a Hyderabad-based sexologist agrees:“Doctors should not interfere in a patient’s personal life. But sadly, some do. A few are open-minded. They do not care whether the patient is married or not. We do at times have to ask about how frequently they have sex to ascertain the cause.”

Kalpana believes the rigid cultural restrictions and undue secrecy about anything related to sex are what makes patients uncomfortable sharing sexual health issues with their doctors.

Prapti (name changed), a 21-year old second year engineering student says: “Ï had  quite a few relationships, and faced initial problems like bleeding and pain during sex. I sometimes lose interest while having sex, due to this immense pain in the vagina.”

But she does not want to consult a doctor: “I prefer advice from friends. At least, they will not judge me.” She remembers the time she had to consult a doctor two years ago, when after having sex, the pain persisted for a whole day.

“The doctor did not even try to explain the reason. I kept asking her whether it was anything serious. But she deliberately chose to ignore me. Later I heard her murmur ‘this generation….uff’! When I shared this with my friends, I realised they too had been in similar situations.

According to Kalpana, only ten percent women come forward to consult a doctor for sexual well-being, of which the majority are planning to get married soon and want to get themselves checked for infection and related advice.

No woman ever goes to the doctor for this, unless it is absolutely avoidable. Not just unmarried women, but even married ones are ignorant in this regard. Young unmarried women are only more hesitant to ask or seek medical help, fearing society and parents, she says.

“Both married and unmarried women are not comfortable. They mostly come with their partners. To make them feel comfortable, we talk to the women alone. After a while, they open up about their problems.”

She also claims that 20% of women who suffer from vaginal infection like UTI and rashes after marriage too feel shy to discuss it with the doctor: “Men seem more comfortable discussing their sexual problems. 90% of our patients are men. But they tend to come alone.”

That was not the case with Jayesh (name changed), a 27-year old. He used to earlier hesitate to talk about his sexual health: “It was only a year back that I consulted a doctor for premature ejaculation, something that I suffered from the age of 23. I used to think if my friends get to know, they would make fun of me.”

The common issues that men in the age group of 18-80 are premature ejaculation and erectile dysfunction. “Most men confess that they force their wives to use contraceptive pills, as they do not want to wear condoms,” Kalpana says.

Gaurav (name changed), a 29-yearold unmarried man insists that he has never forced his girlfriend to use contraceptive pills, but they do sometimes prefer pills over condoms.

Gaurav who is sexually active does not feel ashamed or uncomfortable consulting a doctor, but that is not the case with his girlfriend: “Four years back, she once started bleeding after we had sex. Honestly, I was clueless how to handle the situation and whom to contact. We did not go the doctor, fearing prejudice.

My girlfriend is not at all comfortable consulting a doctor. She usually avoids going to a gynaecologist, as they ask whether we are married or not. It makes her uncomfortable. It happened a few times with us in Hyderabad. That’s why sometimes she prefers to use emergency contraceptive pills rather than consult a doctor.”

“Sex jokes are allowed, but people are otherwise shy talking about sex. Parents do not talk freely on the topic. It is still a taboo for Indian society,” Gaurav remarks.

When Preeti (name changed) -who is now doing an event management course- was in her final BCom year, she led an active sex life:

“I went for a party and got drunk. That night my friend and I had sex. I did not then realise that we had forgotten to use a condom. After missing my periods, I freaked out. I was confused and went to see a doctor. They first asked if I was married. I lied.”

She also admits to feeling uncomfortable while buying I-pills, condoms or pregnancy test devices: “Once a medical shopkeeper asked whether it was for me, with those around giving me judgmental looks.”

Fearing societal disapproval, several unmarried women tend to take medications, after consulting the internet.

“They go to medical stores or send their partners to buy medicines without consulting a doctor. Emergency contraceptive pills have several side-effects like, dizziness, vomiting etc. Some even try to abort through pills, which is life-threatening and can affect their health in the long run,” warns Kalpana.
