Most of us men folk don’t really take the time to notice how our bodies work and so we miss the distinction between the two.

What do ya say we take a minute to discern the difference between an ejaculation and an orgasm? Maybe this will encourage those in my audience who haven’t stopped to notice, to pay a bit more attention to their sexual responses. When we shoot, or dribble as the case may be, a load, or dab as the case may be, of spooge out our cock; this is called an ejaculation. When we have undulating waves of pleasure that seems to rise up from deep inside our pelvis; this is an orgasm. There are big ones and smaller ones. Obviously, these two responses — ejaculation and orgasm — often happen simultaneously. In fact, the muscle contractions of the one often add to the intensity of the other. But still they are independent of one another.

Another thing, a guy can ejaculate and orgasm without an erection. Just because there’s no stiffy doesn’t mean the nerve endings are disconnected. Those of you who follow my reviews may recall that way back in October 2007, when I participated in my first review; I mentioned I was having some prostate problems. I’ve been calling attention to that issue ever since. Well, earlier this year, I was diagnosed with prostate cancer and soon there after I went under the knife. I had a radical prostatectomy, which removed my prostate gland as well as and some of the surrounding tissue.

Like my Dr Dick Review Crew colleague, Angie, who was diagnosed with lymphoma over a year ago, I’ve been struggling to regain a sense of my sexual-self post surgery. No one; not my doctors, not my nurses, not anyone in the cancer support group I attend ever talks about sex and sexuality post diagnosis and treatment. It’s criminal really.

The surgery impacted every aspect of my sex life — with my myself, with my wife and the periodic connections I used to have with some of my men friends. Thank god I’ve been able to count on Dr Dick to help me through this, because if I had to do this alone I don’t see how I would have made it.

I want to repeat something Angie said. “There is precious little information about sex and sexuality available to cancer survivors. No one seemed to be capable of speaking clearly and unambiguously about how a cancer diagnosis and treatment impacts a person’s intimate life. This conspiracy of silence has got to stop!”

Since the surgery I haven’t had a full erection. Dr Dick tells me that I may regain that capacity with time. I also no longer have an ejaculation. That Dr Dick tells me is gone for good. That really bums me out, but I can still have an orgasm, so I’m really happy for that. Dr Dick gave me some exercises to help me with the arousal phase of my sexual response cycle, mostly it masturbation sorts of things. He also suggested that rather than going without partnered sex, I could try a strap on. Hell, I didn’t even know there were strap ons for men. As you can see, I had a lot to learn.

Of course there’s always the possibility that the woman in question is pre-orgasmic — that she’s never had an orgasm so she may think that this is how it’s done. How sad is that?

You know how there’s little mystery about a dude’s pleasure — he gets it up, he gets it on and he gets it off — pretty cut and dried. And there’s often the evidence of his pleasure in the form of a pool of his own jizz. Although I hasten to add that an ejaculation is not the same thing as an orgasm, but it is rare to have an ejaculation that isn’t somewhat pleasurable.

Again, the male sexual response cycle is pretty obvious — he gets wood. For a woman the analogous response is she gets wet. If your lady has a wet pussy, you may be on the right track. Of course lots of women don’t lubricate all that much, so you might not want to rely on this evidence alone. In this instance you might look for the secondary signs of arousal – these are pretty much the same in both women and men. Our pupils may dilate, our skin may flush, our nipples may erect, our breathing may increase to a pant, our heart rate will defiantly increase and our toes might curl. Not all of these will happen every time to every person, but you get the drift, right?

In terms of the big “O” men and women differ in many respects, but there are some commonalities. Both women and men can have “mini orgasms” as well as “major” screamin’ memes. Us men folk tend not to pay too much attention to the little tremors, because we know the big one is coming. Women, on the other hand, have less certainty that the big “O” will show up, so they tend to be more aware of the mini ones. Regardless of the gender a good portion of an orgasm’s physiologic response is the same in all of us. Muscle contractions, specifically the PC (pubococcygeus muscle) in both women and men and vaginal and uterine muscles in women signal orgasm. If the chick you’re bumping has a wild orgasm every single time, I think she’s faking it. If she cums no matter what you’re doing to her, I think she’s faking it. Sex Therapy—What Is It and Who Needs It? – Part 1

I’m often asked about my work as a sex therapist. I’m surprised at how few people have any sense of what a sexologist does. While I can’t speak for all my fellow therapists, I can tell you a bit about my own practice.

Most of the work I do is Cognitive Behavioral Therapy (CBT): short-term, goal-directed and personally liberating (I don’t believe this kind of therapy should become a lifestyle). Basically, I suggest that people with sexual issues change the behaviors that contribute to their problems as a surefire way to solve them. I try to give my clients all the tools they need to successfully work things out on their own once the therapeutic intervention is over. This approach doesn’t fit everyone; however, 99.9 percent of the people I work with respond positively.

I encourage my clients to give themselves permission to investigate their sexuality. This in turn assists them in taking charge of making themselves feel better and/or perform better. And as soon as they do, they almost immediately have a greater sense of wellbeing. Like they say, nothing breeds success like success.

Once we identify an area of concern, my client and I create a plan of action for them to implement. I believe the more an individual is part of their own healing process, the more productive that process will be.

Sadly, I find that fewer and fewer people are willing to give their sexual issues the attention they deserve. Rather than investing the time and energy to get to the bottom of their issues, many opt instead for the quick fix—the “Give me a pill for that” mentality. They’re often unwilling to make the necessary lifestyle changes to actually solve their problems. For example, I encounter people who are eating themselves to death, or abusing alcohol or drugs. Of course they have the accompanying sexual response issues—erection problems for men and arousal concerns for women. They may desperately want to resolve these issues, but without committing to any change in behavior—i.e.: “I want my erection back, but I won’t stop drinking”—such interventions almost always ends in disappointment.

Sexual dysfunction of one sort or another is the issue I see most recurrently in my practice, although the reason why a client reaches out varies. Sometimes an individual’s tolerance level peaks, and they finally decide to do something about an issue that may have been smoldering for years. Sometimes it’s a partner who brings in their proverbially “broken” partner, telling me to “fix him/her.”

Couples often seek sex therapy together, as sexual problems tend to be more obvious within relationships. However, by the time the couple comes for therapy, the issues have most likely been plaguing them for some time. The relationship often comes close to ending before the couple agrees to address the problem. For example: Say a guy brings his wife in because she’s “frigid,” whatever that may entail. They’ve been married for X-number of years, and he’s finally had it. She, on the other hand, doesn’t want to be in therapy, because she doesn’t really think there’s anything wrong with her. She just doesn’t want to have sex anymore, and she doesn’t want to discuss it. Period.

This is a difficult way to start therapy. Resentments are high and frustrations rage. If the couple does continue, we usually discover that there’s also something desperately wrong with the husband. Inevitably, we ascertain that he’s an ineffectual lover—and his inability to pleasure his wife is the root of her “problem.” It’s often painfully clear that he knows little (if anything) about his wife’s sexual needs or desires. Meanwhile, the wife has never had permission to know her body, so she’s unable to help or direct him. As you can imagine in a case like this, there’s a load of remedial sex education that must come before anything else is resolved.

Couples also seek therapy when one spouse has cheated on the other. The “cheat-ee” declares, in no uncertain terms, that this therapy is the last-ditch effort before “the end of the road.” Often in such cases, it’s too late for a successful intervention, because each partner is so angry and shamed that the chance of turning the situation around is slim. Sometimes the best we can do is end the relationship with as little acrimony as possible.

In difficult couple counseling situations like this, my first effort is to get the couple to disarm. There will be no sex therapy—and God knows there is a need for sex therapy—until there is some semblance of peace between partners. If we don’t establish at least a small bank of goodwill, our efforts are doomed.
