5 Ways to Have Healthy Sex When You Have Diabetes

— Here’s how to continue enjoying intimacy

Age and the passage of time usher in many changes. But in all my years practicing medicine, one thing remains a constant: My patients want to continue having a fulfilling and pleasurable sex life, even if they’re dealing with health problems or chronic illness. One disease that can take a heavy toll on a couple’s sex life is diabetes. Fortunately, there are ways to manage it and reignite your sex drive. Don’t despair! Here’s how you can do it.

The role of glucose in your sex life

People with diabetes are more prone to sexual dysfunction than the rest of the population, according to the American Diabetes Association (ADA). Both men and women may experience little or no sex drive due to poor management of their diabetes. But only 47 percent of men and 19 percent of women with diabetes discuss this issue with their doctors, according to a study published by Diabetes Care. Controlling blood sugar levels with medication and regularly monitoring these levels is essential to maintaining a healthy sex life.

But even if blood sugar is kept under control, men and women with diabetes may feel sexual desire but have difficulty becoming physically aroused.

And a failure to control blood sugar will eventually lead to blood vessel and nerve damage, which can prevent arousal.

How does diabetes affect your sex life?

There are many reasons why people with diabetes lose their sex drive or their ability to orgasm. Obesity, high blood pressure, sleep apnea and depression are conditions that often accompany diabetes and can compromise your libido. Some of the medications used to treat these conditions can also adversely affect your sex life. Some treatments for high blood pressure, for instance, can cause erectile dysfunction.

According to a study published in the International Journal of Environmental Research and Public Health, the sexual disorders of patients with type 1 diabetes are directly linked to depression and are less severe in those who accept their diabetes. These disorders also affect men more than women, and they take a greater toll on patients with blood glucose levels above 6.5 percent.

Erectile dysfunction in men with diabetes

If a man’s blood vessels don’t function properly or he has a blocked artery, his penis will not receive enough blood flow to get an erection. Men over 50 with type 2 diabetes are 11 times more likely to experience erectile dysfunction than younger men, according to a study published in the Caspian Journal of Internal Medicine.

Men with diabetes-related erectile dysfunction are also at greater risk of developing Peyronie’s disease, where a type of scar tissue known as plaque forms under the skin of the penis. This causes a curvature that can make erections painful, difficult or impossible.

Sexual problems in women with diabetes

Nerve damage in women can cause vaginal dryness, a condition that is not unique to people with diabetes but, according to the ADA, is twice as common in this population. The neurovascular system plays a necessary role in arousal and orgasm. If the small nerves aren’t working properly due to damage from poorly controlled blood sugar, a woman can have sensation problems. The clitoris needs optimal blood flow and sensation to become engorged enough to achieve orgasm.

Women with diabetes are also at greater risk of urinary tract and vaginal infections, which can make sexual intercourse painful and unpleasurable. Middle-aged women with diabetes who take insulin are 80 percent more likely to have difficulty reaching orgasm than women without diabetes, according to a study published in Obstetrics & Gynecology.

How can you maintain a healthy sex life?

Here are some practical suggestions that anyone can follow:

1. Eat right

Eating healthy can make all the difference. Non-starchy vegetables, such as broccoli and carrots, and whole grains can help stabilize your blood sugar and give you the energy you need to have sex. These foods are also rich in fiber and nourish your gut microbiome — the bacteria and other organisms that keep your gut healthy. A snack before sex will help boost your stamina and keep your blood sugar under control.

2. Exercise

Sex, like any other form of exercise, requires energy. So how can you be prepared? Activities such as weightlifting and Zumba classes can help you build stamina and have more energy in bed. I recommend at least 150 minutes of exercise per week, which can include walking, swimming or even gardening. Exercise also makes the body more sensitive to insulin, which helps it control insulin levels more efficiently.

3. Take your medications

Strictly follow your doctor’s instructions and make sure you take your medications. If you have diabetes, controlling your blood sugar starts with taking your prescriptions. Talk with your doctor if you suspect that a medication prescribed to treat other conditions may be interfering with your sex life, so that you can work together to look for alternatives. I also recommend buying a weekly pill organizer, which has been shown to help increase compliance with medical treatment.

4. Manage stress

Learning that you have diabetes can be upsetting and can affect your mental health. Learn how to calm your mind and body in stressful moments, such as during a sexual encounter. Brush away negative thoughts. Be grateful for — and focus on — enjoyable moments. Meditation can be very helpful. Research shows that meditation may strengthen the parts of the brain that help you remain calmer in stressful situations. But my favorite tool for reducing stress and anxiety is cognitive behavioral therapy; check with your doctor to see if this is a good option for you.

5. Be consistent

Consistency is a bridge between dreams and reality. Follow these suggestions daily. Take it gradually at first. Don’t reach for gimmicky or drastic solutions; just stick to the basics and you’ll get results, especially over time. Remember that the key is consistency, not perfection.

What Your Penis Says About Your Health

— Changes in penis performance or appearance may signal heart issues, diabetes and more

Your penis serves some big roles in your body. Of course, it houses a drainage system that allows your body to get rid of urine. It’s also a key player in the reproductive process and the act of making whoopie.

But did you know your penis also offers a window to your health? It turns out the performance or appearance of your penis can provide clues about what’s happening to you physically and mentally.

So, what secrets can the appendage reveal? Let’s look at six potential learning opportunities with urologist Ryan Berglund, MD.

1. Heart health

Erectile dysfunction, or the inability to get or maintain an erection, isn’t an unusual occurrence. Your penis may just decide to not cooperate at times for a multitude of reasons, many of which are no big deal.

But if you consistently have trouble getting or keeping your penis up, it might signal heart disease and blood flow issues.

A 2018 study linked erectile dysfunction (ED) to increased risk of heart attack, cardiac arrest and stroke. Dr. Berglund notes that almost two-thirds of people who’ve also had heart attacks also experience ED.

“Erectile dysfunction, particularly if you’re younger, should be regarded as a warning sign for heart disease,” he adds.

2. Diabetes

Difficulty getting or maintaining an erection also may signal the onset of diabetes, which can damage the nerves, blood vessels and muscle function that work in tandem to get your penis up.

Research shows that someone with diabetes is three times more likely to report instances of ED. In addition, ED often occurs 10 to 15 years earlier and is more severe in those with diabetes.

3. Mental health issues

The mind plays a very large role in the function of the penis, says Dr. Berglund. Psychological issues such as depression, anxiety and stress can lower sex drive and keep your penis from performing as it should.

Relationship troubles can hinder operations below the belt, too. Ditto for alcohol, smoking and drug use.

4. Scar tissue

Having intercourse with a less-than-firm erection can damage your penis and lead to the development of Peyronie’s disease, a disorder in which scar tissue within the penis causes a curvature.

The condition can lead to a noticeable bend in your penis. A curve greater than 30 degrees is considered severe. The loss of length or girth is possible, too.

ED can cause more flaccid erections that increase your risk of Peyronie’s disease. Talking to a healthcare provider about difficulties getting a hard erection and getting treatment could reduce your chance of sustaining the injury.

5. Infection

Lumps and bumps aren’t unusual on a penis. Blood vessels, pimples and pearly penile papules (small, pearl-like bumps) are just a few of the things you might notice on your penis, shares Dr. Berglund. In most cases, they’re nothing to worry about.

But how can you tell if there’s something more serious going on?

If the bump is painful or there’s an open or weeping sore, get it checked out ASAP. It may be a sexually transmitted infection such as herpes or syphilis. Less pain but lots of itchiness could signal genital warts or molluscum contagiosum (a viral skin infection).

6. Cancer

A discoloring of your penis along with painless lumps, crusty bumps or a rash could be a sign of penile cancer. Symptoms typically appear on the penis head or foreskin and should get checked by a medical professional.

The rate of penile cancer is relatively low in the United States, at 1 in 100,000. But it’s much more common in Africa, Asia and South America.

It’s important to pay attention to what’s happening downstairs. Changes in the performance or appearance of your penis may be a sign of a larger health issue.

Let’s Talk about Sex and Diabetes

Though certain sexual disorders are well-understood in men with diabetes, we know a lot less about the prevalence, impact, and management of sexual dysfunction in women with diabetes. At the ADA Scientific Sessions, Dr. Sharon Parish gave a broad overview of what we do know about this topic.

Dr. Sharon Parish, professor of medicine, clinical psychology and professor of clinical medicine at Weill Cornell Medicine, delivered a fascinating presentation on the third day of the 82nd ADA Scientific Sessions that included a broad overview of sexual disorders and dysfunction in women with diabetes.

What sexual disorders do women with diabetes face?

There are a number of sexual disorders that can affect women and women with diabetes specifically. These include hypoactive sexual desire disorder (HSDD) (reduced sexual desire and motivation), female sexual arousal disorder (reduced sexual arousal), and female orgasm disorder (reduced frequency, intensity, or pleasure of orgasms, and/or delayed, spontaneous, or premature orgasms), among many others.

How common is sexual dysfunction?

The prevalence of these conditions is disheartening. Research shows that in sexually active women with type 2 diabetes, as many as:

  • 50% experience desire problems
  • 34% experience arousal problems
  • 36% experience lubrication problems
  • 36% experience orgasm problems

More recent data shows these rates may actually be slightly lower, and there are differences with type 1 vs. type 2 diabetes. Women with type 1 diabetes having a greater prevalence of sexual dysfunction, including decreased desire, lubrication, and arousal. In women who do have sexual dysfunction, there are also higher rates of diabetes distress, impaired emotional well-being, and anxiety.

The reasons these conditions show up more prominently in women with diabetes could include hormonal reasons, infections, hyperglycemia that affects vaginal lubrication, neurological damage, and increased rates of mental health conditions like depression.

What are the risk factors?

Risk factors for these sexual dysfunctions include older age, obesity, smoking, higher A1C, and longer duration of diabetes. Interestingly, depression and marital status are significant predictors of sexual dysfunction in women.

The importance of screening for sexual dysfunction and reducing stigma

Parish stressed that screening is key, but that these conversations should be initiated by healthcare providers by asking open-ended questions. “Have them tell you a story, ask follow-up questions,” she said.

And as a person with diabetes, being honest with your healthcare provider about how diabetes affects your sex life can help them help you. If they don’t bring the topic up, and you are comfortable, initiate the conversation yourself. This can help normalize talking about sex and reduce the stigma associated with these conversations – all people deserve to have a healthy and fulfilling sex life.

Treatment options for some sexual disorders in women with diabetes

For women with HSDD, Parish broke down three treatment options. If you have this condition, ask your healthcare provider if any of these may be available to you.

For pre-menopausal women, Flibanserin could elevate hormones in your brain that lead to sexual desire and Bremelanotide (an injection taken on-demand) can increase desire and decrease distress. Though there is less research in the area, there is some evidence that testosterone injections given off-label could moderately improve desire in post-menopausal women.

In addition, if the root cause of HSDD is determined to be tied to a psychological or relationship/lifestyle issue, counseling, cognitive behavioral therapy, or psychotherapy could also be good options for treatment.

Finally, Parish explained some signs and symptoms of vulvovaginal atrophy (VVA) and genitourinary syndrome of menopause (GSM) and treatment options. These conditions, which occur post-menopause, can lead to loss of elasticity, soreness, dryness, irritation, and burning. They may be able to be treated with lubricants and moisturizers or low-dose vaginal estrogen.

Sexual health is an important part of your overall health. Talking to your healthcare team about how diabetes affects your sex life, and finding ways to address the root causes of any issues you notice, could improve not only this area of your life but also your emotional and mental well-being. It’s also important for healthcare providers to help initiate these conversations in their clinics.
