My children’s first interactions around sex and sexuality are actually taking place in our home right now. I’ve worked hard to establish where we live as a safe place for them to grow, make mistakes and learn from them, and to inquire about life. It’s why I made the choice early on in their lives to make sure that they learned about sex from me and from their dad, and that in teaching them about sex, we taught our kids to be sex positive. As much as people warned me that the conversation around sex is awkward between a parent and child, I didn’t let the fear of being uncomfortable keep me from taking about sex with my 3- and 2-year-old children.

I’m sure that talking to a 3 year old and a 2 year old about sex sounds like it’s a bit young, but I feel like that’s because we’re so used to framing the sex conversation around the “birds and the bees” conversation. When I was growing up I never had that conversation with my parents and had to frame my own ideals about sex and sexuality through experience and age. I didn’t want that for my children, though. So I felt that a toddler age was actually a wonderful time to start talking to them about how to love their bodies and how to appreciate them. I felt like the intro into sex isn’t about diving head first into questions like “where does the penis go?” and “what is the purpose of the vagina?” I wanted to give my kids a foundation for understanding and respecting their bodies before I ever taught them how about the intimacy shared between two people.

More than anything, I wanted my kids to understand as soon as possible how to love themselves, to understand consent, and to respect others’ bodies. I believe that sex positivity isn’t just about the act of having sex, it’s also about learning that the experience starts with you and will eventually (if you choose) include others.

By the time I was 18, I had disassociated myself from my body because of how my parents talked about it. now I had the chance to do things differently.

My upbringing kept me from understanding what sex was. My parents sex hidden, far above my reach. I was told we’d open that box when I was old enough, but only when I was was getting close to marriage. I found this strange — even at 10 years old. I would look sex up in the dictionary and in the encyclopedia. I often wondered what sex was and what was so special about it — why was it something only adults could understand? I’d hear my friends talk about boobs, about liking boys, and wonder if I’d ever feel comfortable enough to be naked around another person I liked. At the time, the thought horrified me.

I was uncomfortable with my body. I didn’t understand what was happening to it, or why I was suddenly getting hair under my armpits and on my vagina. My parents were constantly telling me to “be modest,” and I felt so much pressure and responsibility to look and behave and act a certain way. By the time I was 18, I had disassociated myself from my body because of how my parents talked about it. now I had the chance to do things differently.

When I was 18, I was in love and I had sex for the first time. It was amazing, and I had no idea why I’d been so afraid and so ashamed. I was raised Christian and was taught to believe that sex before marriage was shameful. But after having sex for the first time, I didn’t want any forgiveness. I simply wanted to keep having sex, without feeling guilty because of it. After I’d gotten married to my then-husband and had two kids, I looked back on my own sexual experiences and realized that I didn’t want my children gaining their sex education from the world around them without some input from me. I didn’t want them feel ashamed of the fact that they liked having sex or pleasuring their bodies. I wanted my kids to know that they could always come and talk to me, that I would always support them.

I tell them dressing my body in things that make me feel confident makes me feel empowered, as if my body hold some kind of magic. They love that. So do I.

So I started to talk to them about celebrating their bodies when they were young. And because of that, I had deeper conversations with myself surrounding my own sex positivity. I had some sexual trauma in my past, which has always made it a bit difficult for me to grapple with wanting to be sexual and carving out safe spaces to practice having sex. I made changes in my personal life: I was more vocal with myself about my needs and wants, then with partners. It helped me shape the conversations I’d have with my children about how they can and should voice what they want, not with sex because that’s still a ways off, but when interacting with others. I wanted them to learn and understand the power of their own voices. I taught them to say, “No, that’s not something I would enjoy,” or “I would really like if we did this” in their everyday lives, knowing that these lessons will help them in their sexuality later on. We’ve focused on how important it is for them to speak up for themselves and to advocate for themselves.

Another thing we do in our house is walk around naked. I used to shy away from showing parts of my body, like my stomach or my thighs. I have stretch marks and cellulite — both things I’ve been told aren’t “sexy.” My kids, however, could care less about whether or not my body is sexy enough, because they just like how soft my body is. It’s soft for cuddling and for hugging, two things that are very important to them. My kids move so confidently with their bodies, both with clothes on and with clothes off. My daughter’s favorite thing is to stand in front of the mirror and compliment herself. She’s actually inspired me to do the same. I’ve taken up the practice. They’ve seen me in some of my lingerie, and tell me it’s beautiful. They don’t know that lingerie is “just for sex” or that it’s something I should feel wary of other people seeing. Instead, I tell them dressing my body in things that make me feel confident makes me feel empowered, as if my body hold some kind of magic. They love that. So do I.

I watch them be confident in their bodies. I watch them say “no” strongly to each other, and to others, and most importantly, I watch them hear and respect each other.

My kids are 6 and 7 years old now, and we’ve talked about what sex is. The conversation has changed as they’ve grown up. They understand that sex is a beautiful act, one that mostly happens when people are naked. They don’t really care to know more yet, but I watch them be confident in their bodies. I watch them say “no” strongly to each other, and to others, and most importantly, I watch them hear and respect each other. As a person who is non-monogamous, I’ve shown them that sex and love are not limited to one person. It can be, but it doesn’t have to be. In turn, my children have taught me to respect and be proud of my body. They think it is magic — and I agree.

Lately, the children have been exploring their bodies, which I’ve told them is fine, but it’s reserved for their alone time. I’m trying to make sure that when we talk about our bodies and about sex that we do so in an uplifting, positive way. I don’t want my children to ever question or feel any shame around their bodies or their wants. I want to equip them with the right knowledge so that they’ll be able to enjoy. Most of all, I want them to be happy.

Doctors urged to advise patients about risks of abstinence-centric sex education

American Academy of Pediatricians’ new report is the clearest denouncement of the failures of not talking about STIs and pregnancy prevention

The country’s largest organization of pediatricians entered fraught political territory on Monday, with a call for doctors to use their time with patients to combat the potential health consequences of abstinence-centric sex education.

In a new report, the American Academy of Pediatricians (AAP) issued its clearest denunciation yet of sex education programs that fail to offer comprehensive information on topics such as sexually transmitted infections (STIs) and pregnancy prevention.

“This is the mothership telling pediatricians that talking about sex is part of your charge to keep children and adolescents safe,” said Dr Cora Breuner, a professor and pediatrician at Seattle Children’s research hospital and the report’s lead author.

“These guidelines give pediatricians in communities where people might say, ‘We don’t want you talking to our kids about this stuff,’ permission to say, ‘No, I can talk about this, I should talk about this, I need to talk about this.’”

The report is broadly a call for pediatricians to help fill in the gaps left by the country’s patchwork sex education programs. It urges pediatricians to teach not only contraception and the benefits of delaying sexual activity, but to cover topics such as sexual consent, sexual orientation and gender identity with school-aged children who may not receive any information in the classroom and involve their parents.

But the authors single out abstinence-heavy education, which sometimes excludes information about contraceptives, as a key concern for doctors looking to help adolescent patients avoid sexually transmitted infections and unintended pregnancy. As a result, it is likely to fuel an already contentious debate.

Groups that have advocated for sex education to emphasize abstinence instantly found fault with the new guidelines.

“A health organization like the AAP should not be affirming a behavior that can compromise the health of youth,” said Valerie Huber, the president of Ascend, a group that promotes abstinence-centric sex education and advocates for federal funding. The group was formerly known as the formerly the National Abstinence Education Association.

“They recommend ‘responsible sex’ for young adolescents. Exactly what is responsible sexual activity for adolescents? … The science is clear that teens are healthier when they avoid all sexual activity.”

Moreover, Huber said, programs that “normalize teen sex” are unpopular with many parents.

“Most communities do not support the type of sex education they recommend,” she said.

Still, others embraced the report as bringing the AAP’s recommendations more in line with the reality.

“This is a fantastic move,” said Chitra Panjabi, the president of the Sexuality Information and Education Council of the United States (SIECUS), a research group that supports comprehensive sex education. “It’s really important that our medical providers are standing up and saying, hey, the youth in our communities are coming to us because they’re not getting the information they need. And so we need to step in.”

The US does not enforce national standards for sex education and schools in many states are not required to teach it. Across the country, SIECUS estimates, only 50% of high school students receive sex education that meets the recommendations of the federal Centers for Disease Control and Prevention. The other half of students receive anything from an incomplete sex education, to education that emphasizes abstinence, to abstinence-only education, with a focus on delaying sex until heterosexual marriage.

In February, Barack Obama proposed a budget for 2017 that eliminated the $10m the department of health and human services spends on abstinence-only programs every year. But funding continues to flow to those programs from other sources. Title V, an abstinence-only program, allocates $75m a year to abstinence-only programs, money that states match by 75%.

In the last quarter-century, programs emphasizing abstinence as the optimal way to avoid pregnancy and STIs have received more than $2bn in funding from the federal government. Comprehensive sex education, by contrast, has no dedicated federal funding stream.

“It’s a political climate where people don’t want to talk about these issues,” said Breuner. “But it makes our job so much harder when we cannot coordinate our efforts with the schools. It takes time away from the other safety issues we need to be discussing. Don’t smoke weed. Don’t text and drive.”

Recently, two major surveys of existing research on sex education concluded that there was no evidence or inconclusive evidence to show that abstinence-centric programs succeeded in delaying sexual activity. One of the surveys found that comprehensive sex education was actually more effective than abstinence education at delaying sexual activity in teens. (Ascend points to select studies which show the opposite.)

A long-term study found that teens receiving abstinence-only programs were less likely to use contraceptives or be screened for STIs, although rates of infections were not elevated.

The studies helped compel the AAP to issue its first major guidance on sex education since 2001.

“It’s important for pediatricians to have the backing to say, ‘Look, I can’t support telling this stuff to children,’” Breuner said. “I have to deal with the aftermath, which is a 15-year-old who’s pregnant, or a 16-year-old who has a sexually transmitted infection he’s going to have for the rest of his life.”

Breuner said a number of her patients have suffered consequences from abstinence-only education. Many of them are pregnant teenagers and girls who, in the absence of accurate information, came to believe in common myths about pregnancy prevention.

“They’ll say, ‘I thought you couldn’t get pregnant when you were having your period,’ or, ‘I thought it took two or three years after you get your period to be able get pregnant.’ It’s heartbreaking, because I know with education, this could have been prevented.”

5 everyday ways to teach your kids about consent

Sexual consent can be tough to explain to young kids. But this psychotherapist has some advice.

My daughter and I are waiting in the exam room for the pediatrician. We are here for her annual wellness checkup.

And from the moment our pediatrician walks through the door, she is all about focusing on my daughter. She looks my daughter in the eyes and kindly greets her. She shakes her hand. She addresses my daughter with her questions. She explains what we’ll be doing today.

And the pediatrician asks for consent. She asks, “May I listen to your heartbeat now?”

As a psychotherapist, I’m tuned into the ways in which our pediatrician is communicating this message to our daughter: “I regard you as a human being,” “You matter,” and “Your body is to be regarded.”

This is very different from a routine doctor’s visit at a different office I had many years ago with my son, though.

During that visit, the pediatrician rushed through the exam. He didn’t look at my son or address him. The nurse came in with the immunizations and said, “Hold him down. It’s better if we do this fast without him knowing what’s coming. He won’t remember this.”

As a mom, I knew my son. As a body-centered psychotherapist, I knew that his nervous system would remember this experience. And I knew that conditions like that can cause an experience to be traumatic for a young person. “No,” I said. “I know what my son needs. I need to talk to him first and explain what we are doing.”

That day, we didn’t rush, we didn’t surprise him, we didn’t hold him down, and we didn’t give him a treat for “not crying.” I showed my son regard by honoring what I knew he needed.

Parents: Teaching sexual consent to our children begins with us.

Every parent I know wants their child to grow up to be confident, be resilient, feel good about who they are, and show compassion toward others. As parents, we want to communicate: “You matter. Your body matters. Your consent and boundaries matter.”

This is regard, and it begins the moment our children are born. We communicate messages that help our children form their self-concept and sense of self-worth. And they learn how to interact with themselves and others through our regard for their bodies, emotions, opinions, and personhood.

With regard as your foundation, here are five everyday ways you can teach your children about sexual consent:

1. Ask for their consent often.

Last night, my son and I were walking home from the park. I went to reach for his hand, but then I stopped myself and asked him, “Can I hold your hand?” He smiled at me and reached out.

Asking for your child’s permission to touch them or come into their personal space can be this simple. You can ask such questions as: “May I brush your hair?” “Can I have a hug?” and “Is it OK if I hold your hand?”

Does this mean you have to ask for their consent every time? As parents, we want to be intentional about what we are doing and why we are doing it.

Imagine your children as teenagers going out with friends with hundreds — if not thousands — of experiences at home where you modeled consent day after day. They will be more likely to respond to any situation with regard for their bodies, and they will be more likely to regard others’ bodies and ask for consent, too.

2. Teach them that their “no” matters.

A client came to me because she was feeling distant from her 12-year-old daughter. And in working together, we eventually realized that her daughter wanted more regard for her personal space, time, and boundaries.

So she started to look for ways she could ask, rather than demand, that her daughter engage with her. Instead of saying, “Give me a hug goodbye,” she would ask her daughter, “Can I have a hug goodbye?” And on the days her daughter said “no” or her body language indicated “no,” she would say, “That’s cool. If you ever want a hug, I’m here. I love you. Have a great day.”

If you ask to brush your daughter’s hair, and she says “no,” it’s so important to regard her “no.” If you ask to hug your son, and he says “no,” regard his “no,” too. You could reply with, “OK, I respect that. Let me know if you change your mind.”

It’s also OK if your child doesn’t want to hug anyone at all. They can still respectfully greet others with a sincere acknowledgment of “hello.”

When you or anyone else begs or tries to convince a child to change their answer now, they learn to override their inner barometer of what feels comfortable and what doesn’t feel comfortable just to give in to someone who they perceive has more power. Over time, you respecting their “no” teaches your children that their “no” matters.

3. Model to your child that “yes” can become “no” at any time.

Let’s say you are gently wrestling with your young child, and she says, “Stop!” What do you do? You stop. Even if she is joking, you stop and check in with her.

Let’s say you have a group of elementary-school-aged boys over your house, and they are running around with swords and roughhousing. Teach them to pause the game every so often and check in with each other to see if the game is going OK for everyone.

And if you have a tween or teenager? Have “the conversation.” As you share about sexual intimacy based on your family’s values, include communicating to them that the absence of “no” is not “yes.” Teach them that a “yes” can turn into a “no” at any time.

When you model to your child that “yes” can become “no” at any time in everyday experiences, you are sending the message “at any point when you feel uncomfortable or have had enough in any situation, you are to listen to that inner voice. And at any point another person feels uncomfortable and has had enough, you are to respect them and stop what you are doing.”

4. Seek to understand.

This past spring, my daughter announced, “I don’t want to take gymnastics anymore!” I was confused. I thought she loved gymnastics. I had put a lot of thought and effort into finding the right place for her. But instead of saying to her: “Yes, you do! I know you do!” I said, “Tell me about it.”

This opened the door for my daughter to feel comfortable to share what she was feeling and for me to listen to her. I came to understand that actually she loved gymnastics, but what she really loved was doing gymnastics on her own at home and making up routines rather than being in a structured class.

When you seek to understand your child, you communicate the message: “Your opinion matters. Your voice matters. Your feelings matter. And I’m here to listen and be alongside you.” Even if you think your child is playing around or sharing an opinion out of frustration, when you seek to understand, you are connecting to your child with regard.

5. Keep “regard” at the forefront of your mind.

Our children have their own bodies, minds, feelings, opinions, and dreams. Just like adults, our children want to be regarded, listened to, and respected. So ask for your child’s opinion. Speak your child’s name in a way that is regarding. Look at your child when he or she is talking. These are everyday ways that you can communicate the message “You matter.”

We are our children’s first teachers.

The recent Stanford sexual assault case reminded me, yet again, that we have work to do as a culture when it comes to teaching our children about sexual consent.

As parents, it can feel scary to broach loaded and triggering topics like sexual consent. However, these simple, everyday actions can empower us to show regard to our children in our daily lives. And as our children experience our regard in everyday ways, they are more likely to regard themselves and other people’s bodies and integrity, too.

No matter the age of your child, you can support your child being a confident, resilient, compassionate (to self and others) person by choosing to look at, talk to, and be with your child. You can support your child’s future by the regard you show them today.

8 lessons for my sexually uneducated teen self

By what I can only assume was an issue with the timetable I ended up having sex education at least three times during my years of education at middle and high school (yes I went to a ‘middle school’).

And for all their effort I remember being confused, uninformed and altogether none the wiser when the teaching staff tried to inform us about the goings on of the birds and the bees, (a saying I actually still don’t fully understand the significance of. Birds don’t have sex with bees as far as I’m aware).

Having a partner who’s part Dutch and who received (in my opinion) the best sex education in the world, thanks to the Netherlands government, I’m taking the time to look back on my sex-ignorance and highlight some of the key things I’d wished I’d known back then.

1 – Porn is not an accurate representation of real bodies or real sex.

I could quote a load of statistics but I think it’s well enough known that my generation are among the first to grow up in a world where pornography is in such easy reach. I can hardly blame my education for being a little slow on the uptake of something relatively new, but for future sex ed it seems essential to incorporate teaching on how we should perceive pornography as fantasy and not based on real sex lives. It also seems more important to bring parents into sex ed to try and bridge the generation gap that the internet has caused.

2. How to properly check yourself.

I remember plenty of talks on what to do to prevent STIs but I cannot remember ever being told what’s healthy and good and what I should look out for in my own body. I learned more about my own body by visiting my GP for an MOT than I did from a whole series of sex education lessons. Even Youtube provided better sex ed than my school ever did thanks to guys like Riyadh K uploading videos on how to check your testicles for cancer – we were never told that in school.

3. Pleasure is one of the most if not the most important part of sex.

Pleasure was completely missed out of our sex education curriculum. There was such a strong emphasis on the adverse effects of sex and the dangers; the risks of STIs and unwanted pregnancy, that its main purpose was more or less completely ignored. An understanding of the body and pleasure seems essential if you’re going to teach sex ed. There is something intrinsically British about being embarrassed when communicating about our own bodies and all the weird and wonderful things they do. That needs to be swept away.

4. Some men have sex with other men and some women have sex with other women.

As a gay man (well, gay boy at the time) I was excluded from most topics covered by our sex ed. Everything catered to a heterosexual norm and the sex lives of gay people, let alone the relationships of gay people, were left well alone. Thank the lord for Queer as Folk.

5. The specific things you can do as a gay man to help protect yourself.

I only learnt of the real dangers for me as a sexually active gay man through taking some initiative and going to a clinic. I had no clue about hepatitis jabs and emergency HIV treatments and windows of infection. I learned a lot through being able to ask questions of someone I could trust who knows what they’re on about. I also found that going to a clinic completely reversed my expectations which were based on the stereotype of sexual health clinics being sleazy and disgusting. I found it to be a place where I could freely ask all the questions I had which weren’t being met by the teaching at school, (big up Worthing sexual health, woo!).

6. Relationships are a big part of sex education too.

There was so much focus on the physical that the emotional side was almost forgotten. All of the emotional side of things more often than not were put down to hormones. Those pesky hormones were responsible for everything! Nobody attempted to delve deeper into the way we were feeling emotionally and why we were driven to think that the Smiths really did understand us like nobody else did.

7. Consent. A topic that as far as I can remember was not even covered.

The darker side of things including abuse and rape was not touched on, which seems absolutely ridiculous. Teaching consent is essential, especially in an age where pornography is distorting the idea of what is perceived as acceptable and unacceptable in a healthy sexual relationship.

8. Confidence is the most important part of your body image.

In our teenage years we spend so much time obsessed with wanting to look good and fighting Mother Nature who has destined us to be spotty, greasy-haired, squeaky-voiced slobs. Accepting body image and being confident with your own body is probably one of the lessons that comes with age but it certainly would have helped having some reassurances from school forcing our eyes away from the skinny catwalk models and the chiselled muscle men that we were thinking we should look like.

I feel like this may have just turned into a list of failings of our education system. But maybe it isn’t ALL bad and maybe things are changing. If you had a similar experience or if you had a totally different experience of sex ed let me know your thoughts!

8 Sex Positive Things You Can Say To Your Kids That Have Nothing To Do With Sex

“Sex positive” is, I am pleased to note, a term that has been gaining more attention in recent years. A social and philosophical response to repressed, limited, and often judgmental attitudes toward sex and sexuality, the sex positive movement emphasizes that “good sex” is defined as safe, informed, consensual, and whatever else it also is beyond those things is best left up to the people participating in the act. That’s it, and I think that’s awesome. As a parent, I am already doing my best to encourage sex positive attitudes in my children, who are 4-years-old and 19 months — despite the fact that they have absolutely no idea what sex is, and I don’t have plans to get into what it is with either of them any time soon. No, this is not a contradiction, and it’s not hard to do.

What it comes down to is this: Sex positivity rarely exists in a vacuum. It’s usually part of a larger life philosophy that believes all people are entitled to happiness and respect. I have found that there are broad areas of overlap between the body positive and fat acceptance movements, feminism, and the LGBT community. As such, there is so much a parent can say to their child that lays the groundwork for them to have happy, healthy, and fulfilling sex lives (when they’re ready) that don’t necessarily have a thing to do with sex.

So not only does no one get touch you without your permission, but you decide what happens to it. If you teach little kids that this is true in the non-sexual streets, they’ll be more likely to automatically believe it once they grow up and get between the sheets.

Using grown up anatomical terms for all of a person’s various bits and bobs does a couple of sex positive things. It enables your child to talk about their body specifically, which can enable them to be specific and clear if there’s a problem. It also puts “bathing suit area” body parts on the same level as “arm” or “foot,” which we have collectively decided are not parts that require euphemisms. None of our body parts are anything to be secretive or ashamed about. Yes, of course, children should be taught that “vulvas are private” or “only you or a doctor is allowed to touch your testicles,” but that’s not going to be conveyed by giving those parts cutesy names and getting all jittery when someone talks about them.

Kids will ultimately not find non-heterosexual relationships confusing at all (just ask same-sex parents). If a kid has never met a gay couple before, they might be a bit surprised by the idea the same way my kid was surprised the first time he saw purple M&Ms. But after, like, five seconds it’s like, “Oh. So this is the same basic concept as literally any other relationship/M&M I’ve ever encountered in my young life. Cool. Whatever.”

Point is, letting your kids know that anyone can fall in love with anyone else doesn’t have to be a “very special after school special” conversation “when they’re old enough.” Anyone who thinks kids can ever be “too young” to be aware of non-heterosexual romantic relationships, by asserting that, is essentially just admitting, “I think there’s something wrong and bad and dangerous and upsetting about non-straight people.” Do you not feel that way? Then do yourself, your kids, and the world the favor of unburdening yourself of the idea that kids need to be grown up and holding onto something sturdy before they find out that some kids have two mommies. This is something anyone who understands what love and relationships are can understand. This sets the stage for your child to know acceptance is not for a select few, but for everyone.

This is basically one of the central tenets of sex positivity, but, again, doesn’t have to be limited to sexual attitudes. It’s never too early to tell your kids, “Look, different people are made happy by different things. Different people believe in different things. And sometimes those things may seem strange to you… and that’s fine. You don’t have to do what they do any more than they have to do as you say or believe.” I cannot count the number of times a day I have to tell my son, “You do not get to tell your sister how to play with her toys. It doesn’t matter if you think she’s playing with it wrong. There’s no right way to play with a truck.” Some day he might even get it!

“Everyone’s Body Deserves Respect.”

The life blood of the sex positive movement is the idea of mutual respect. Of course, the idea that everyone deserves respect is a core tenet of, like, common human decency and hopefully everyone strives to instill this value in their children regardless of their desire for them to be raised with sex-positive attitudes. So I’ll take it a step beyond “everyone deserves respect” and talk about emphasizing the idea that every BODY deserves respect. Kid bodies, adult bodies, your body, other people’s bodies, disabled bodies, fat bodies, thin bodies, black bodies, women’s bodies, etc., and any intersection thereof. We can’t define a “good body” simply as one that is pleasing to a viewer. A good body is one that enables the person who lives inside of it to do things and be someone that makes them happy. Body positivity is going to be clutch in laying the groundwork for sex positivity.

Even Fewer Kids Are Learning Basic Things About Sex Ed

The United States, which has the highest teen pregnancy rate in the developed world, isn’t exactly known for its top-notch sexual health resources. But a new study suggests that our country’s sex ed has gotten even worse in recent years.

Even fewer teens are now getting basic sex ed information, like formal instruction about how to use birth control, according to researchers at the Guttmacher Institute who compared sexual health data over a seven year period.

The researchers compared data collected by the Centers for Disease Control and Prevention (CDC) during two of the agency’s survey periods: the 2006–2010 survey and 2011–2013 survey. These surveys included questions for teens about whether they ever received formal sex education materials — like how to prevent sexually transmitted infections, how to say no to sex, how to put on a condom, and how to use different methods of contraception — before they turned 18.

In the 2006-2010 survey period, 70 percent of girls and 61 percent of boys said they had received some information about birth control methods. But in the later survey period, those numbers dropped to 60 percent among girls and 55 percent among boys.

As time passed, fewer girls also reported receiving any formal education on how to prevent STDs and how to say no to sex. These declines were particularly acute in rural areas of the country, where teens already struggle with higher rates of unintended pregnancies.

The study’s lead researcher, Laura Duberstein Lindberg, characterized the declines in sex ed instruction as “distressing.” She also pointed out that this data fits into a bigger pattern in the United States. Over the past two decades, the number of teens receiving formal instruction about birth control has been steadily declining, and abstinence-only classes that don’t include accurate information about sexual health have persisted.

“The United States is moving in the wrong direction,” said Leslie Kantor, the vice president of education at Planned Parenthood, the nation’s largest sex ed provider. “Sex education can make a real difference in adolescents’ overall health and well-being. The fact that young people are being deprived of information critical to their sexual health is unacceptable.”

Only 21 states and the District of Columbia currently require sex education and HIV education to be taught in public schools. An even fewer number, 18, explicitly require information about contraception in the classroom. On the other end of the spectrum, 37 states mandate that schools should focus on lessons about abstinence.

There’s a lot of evidence that providing teens with accurate information about sex ed helps them make healthier choices. Sex ed classes are actually linked to a delay in sexual activity — suggesting that, instead of spurring teens to become more sexually active, talking to them about sex actually helps them make more thoughtful decisions about their bodies.

“We need to right the ship, get back on track, and make sure all students receive quality sex education that prepares them to make informed and healthy decisions,” said Debra Hauser, the president of Advocates for Youth, a nonprofit group focusing on the reproductive health issues that are important to young people.

When Did Porn Become Sex Ed?

Conversations between adults and teenagers about what happens after “yes” remain rare.

THE other day, I got an email from a 21-year-old college senior about sex — or perhaps more correctly, about how ill equipped she was to talk about sex. The abstinence-only curriculum in her middle and high schools had taught her little more than “don’t,” and she’d told me that although her otherwise liberal parents would have been willing to answer any questions, it was pretty clear the topic made them even more uncomfortable than it made her.

So she had turned to pornography. “There’s a lot of problems with porn,” she wrote. “But it is kind of nice to be able to use it to gain some knowledge of sex.”

I wish I could say her sentiments were unusual, but I heard them repeatedly during the three years I spent interviewing young women in high school and college for a book on girls and sex. In fact, according to a survey of college students in Britain, 60 percent consult pornography, at least in part, as though it were an instruction manual, even as nearly three-quarters say that they know it is as realistic as pro wrestling. (Its depictions of women, meanwhile, are about as accurate as those of the “The Real Housewives” franchise.)

The statistics on sexual assault may have forced a national dialogue on consent, but honest conversations between adults and teenagers about what happens after yes — discussions about ethics, respect, decision making, sensuality, reciprocity, relationship building, the ability to assert desires and set limits — remain rare. And while we are more often telling children that both parties must agree unequivocally to a sexual encounter, we still tend to avoid the biggest taboo of all: women’s capacity for and entitlement to sexual pleasure.

It starts, whether intentionally or not, with parents. When my daughter was a baby, I remember reading somewhere that while labeling infants’ body parts (“here’s your nose,” “here are your toes”), parents often include a boy’s genitals but not a girl’s. Leaving something unnamed, of course, makes it quite literally unspeakable.

Nor does that silence change much as girls get older. President Obama is trying — finally — in his 2017 budget to remove all federal funding for abstinence education (research has shown repeatedly that the nearly $2 billion spent on it over the past quarter-century may as well have been set on fire). Yet according to the Centers for Disease Control and Prevention, fewer than half of high schools and only a fifth of middle schools teach all 16 components the agency recommends as essential to sex education. Only 23 states mandate sex ed at all; 13 require it to be medically accurate.

Even the most comprehensive classes generally stick with a woman’s internal parts: uteruses, fallopian tubes, ovaries. Those classic diagrams of a woman’s reproductive system, the ones shaped like the head of a steer, blur into a gray Y between the legs, as if the vulva and the labia, let alone the clitoris, don’t exist. And whereas males’ puberty is often characterized in terms of erections, ejaculation and the emergence of a near-unstoppable sex drive, females’ is defined by periods. And the possibility of unwanted pregnancy. When do we explain the miraculous nuances of their anatomy? When do we address exploration, self-knowledge?

No wonder that according to the largest survey on American sexual behavior conducted in decades, published in 2010 in The Journal of Sexual Medicine, researchers at Indiana University found only about a third of girls between 14 and 17 reported masturbating regularly and fewer than half have even tried once. When I asked about the subject, girls would tell me, “I have a boyfriend to do that,” though, in addition to placing their pleasure in someone else’s hands, few had ever climaxed with a partner.

Boys, meanwhile, used masturbating on their own as a reason girls should perform oral sex, which was typically not reciprocated. As one of a group of college sophomores informed me, “Guys will say, ‘A hand job is a man job, a blow job is yo’ job.’ ” The other women nodded their heads in agreement.

Frustrated by such stories, I asked a high school senior how she would feel if guys expected girls to, say, fetch a glass of water from the kitchen whenever they were together yet never (or only grudgingly) offered to do so in return? She burst out laughing. “Well, I guess when you put it that way,” she said.

The rise of oral sex, as well as its demotion to an act less intimate than intercourse, was among the most significant transformations in American sexual behavior during the 20th century. In the 21st, the biggest change appears to be an increase in anal sex. In 1992, 16 percent of women aged 18 to 24 said they had tried anal sex. Today, according to the Indiana University study, 20 percent of women 18 to 19 have, and by ages 20 to 24 it’s up to 40 percent.

A 2014 study of 16- to 18-year-old heterosexuals — and can we just pause a moment to consider just how young that is? — published in a British medical journal found that it was mainly boys who pushed for “fifth base,” approaching it less as a form of intimacy with a partner (who they assumed would both need to be and could be coerced into it) than a competition with other boys. They expected girls to endure the act, which young women in the study consistently reported as painful. Both sexes blamed the girls themselves for the discomfort, calling them “naïve or flawed,” unable to “relax.”

According to Debby Herbenick, director of the Center for Sexual Health Promotion at Indiana University and one of the researchers on its sexual behavior survey, when anal sex is included, 70 percent of women report pain in their sexual encounters. Even when it’s not, about a third of young women experience pain, as opposed to about 5 percent of men. What’s more, according to Sara McClelland, a psychologist at the University of Michigan, college women are more likely than men to use their partner’s physical pleasure as the yardstick for their satisfaction, saying things like “If he’s sexually satisfied, then I’m sexually satisfied.” Men are more likely to measure satisfaction by their own orgasm.

Professor McClelland writes about sexuality as a matter of “intimate justice.” It touches on fundamental issues of gender inequality, economic disparity, violence, bodily integrity, physical and mental health, self-efficacy and power dynamics in our most personal relationships, whether they last two hours or 20 years. She asks us to consider: Who has the right to engage in sexual behavior? Who has the right to enjoy it? Who is the primary beneficiary of the experience? Who feels deserving? How does each partner define “good enough”? Those are thorny questions when looking at female sexuality at any age, but particularly when considering girls’ formative experiences.

We are learning to support girls as they “lean in” educationally and professionally, yet in this most personal of realms, we allow them to topple. It is almost as if parents believe that if they don’t tell their daughters that sex should feel good, they won’t find out. And perhaps that’s correct: They don’t, not easily anyway. But the outcome is hardly what adults could have hoped.

What if we went the other way? What if we spoke to kids about sex more instead of less, what if we could normalize it, integrate it into everyday life and shift our thinking in the ways that we (mostly) have about women’s public roles? Because the truth is, the more frankly and fully teachers, parents and doctors talk to young people about sexuality, the more likely kids are both to delay sexual activity and to behave responsibly and ethically when they do engage in it.

Consider a 2010 study published in The International Journal of Sexual Health comparing the early experiences of nearly 300 randomly chosen American and Dutch women at two similar colleges — mostly white, middle class, with similar religious backgrounds. So, apples to apples. The Americans had become sexually active at a younger age than the Dutch, had had more encounters with more partners and were less likely to use birth control. They were also more likely to say that they’d first had intercourse because of pressure from friends or partners.

In subsequent interviews with some of the participants, the Americans, much like the ones I met, described interactions that were “driven by hormones,” in which the guys determined relationships, both sexes prioritized male pleasure, and reciprocity was rare. As for the Dutch? Their early sexual activity took place in caring, respectful relationships in which they communicated openly with their partners (whom they said they knew “very well”) about what felt good and what didn’t, about how far they wanted to go, and about what kind of protection they would need along the way. They reported more comfort with their bodies and their desires than the Americans and were more in touch with their own pleasure.

What’s their secret? The Dutch said that teachers and doctors had talked candidly to them about sex, pleasure and the importance of a mutual trust, even love. More than that, though, there was a stark difference in how their parents approached those topics.

While the survey did not reveal a significant difference in how comfortable parents were talking about sex, the subsequent interviews showed that the American moms had focused on the potential risks and dangers, while their dads, if they said anything at all, stuck to lame jokes.

Dutch parents, by contrast, had talked to their daughters from an early age about both joy and responsibility. As a result, one Dutch woman said she told her mother immediately after she first had intercourse, and that “my friend’s mother also asked me how it was, if I had an orgasm and if he had one.”

MEANWHILE, according to Amy T. Schalet, an associate professor of sociology at the University of Massachusetts, Amherst, and the author of “Not Under My Roof: Parents, Teens, and the Culture of Sex, ” young Dutch men expect to combine sex and love. In interviews, they generally credited their fathers with teaching them that their partners must be equally up for any sexual activity, that the women could (and should) enjoy themselves as much as men, and that, as one respondent said, he would be stupid to have sex “with a drunken head.” Although she found that young Dutch and American men both often yearned for love, only the Americans considered that a personal quirk.

I thought about all of that that recently when, driving home with my daughter, who is now in middle school, we passed a billboard whose giant letters on a neon-orange background read, “Porn kills love.” I asked her if she knew what pornography was. She rolled her eyes and said in that jaded tone that parents of preteenagers know so well, “Yes, Mom, but I’ve never seen it.”

I could’ve let the matter drop, felt relieved that she might yet make it to her first kiss unencumbered by those images.

Goodness knows, that would’ve been easier. Instead I took a deep breath and started the conversation: “I know, Honey, but you will, and there are a few things you need to know.”

Dr., I’ve been having sex with a friend who is 19 and he expressed that he had feelings for me at a time when I didn’t have feelings for him. We got into an argument and didn’t talk for a few weeks and I noticed that I missed him and now we’re talking sort of, but I feel that now its more physical than anything, on his part. I think I do have feelings for him, but the only time he attempts to talk to me is when he is horny. The biggest issue is that I haven’t come out and I don’t feel it’s the right time for me to, but I think if I did, it would help things between me and him. Could you give me any advice?

Yep, pup, you sound pretty confused all right. But then you can hardly be faulted for your bewilderment, being as young as you are.

But if the truth be known, your youth has very little to do with it. The first thing you oughta know is that people grapple with these weighty concerns all their lives. I don’t know anyone who has all this emotional stuff pinned down. Because just as soon as ya think ya have it all figured out, life throws you a curve ball.

You will find if you haven’t done so already, that there is often a very fine line between love and desire, passion and intimacy and lust and affection. You should also know that sometimes, maybe even most of the time, we are out of sync with the person we are involved with. In the immortal words of Stephen Sondheim — either we are at last on the ground, while they’re in mid air. Or they keep tearing around while we can’t move. Since the beginning of time, most songs, epic love stories and romantic poetry have wrestled with the shifting fortunes of matters of the heart…and the groin.

So if you ever do make sense of all of this in your life, just wait a minute. The whole kit and caboodle will surely collapse like a house of cards around your head. That’s what I’ve found in my life anyhow. I still struggle with all of this and I’m fuckin’ old, don’t cha know.

Actually, I believe that the tension between love and lust is the very thing that gives life its spice. The only thing I’ve been able to discover after all this time is that there isn’t a code to break, just a toboggan ride to be had.

Which bring me to the second comment I want to make. I know that a lot of young people…your age and even younger…are experimenting with sex, and that’s not a bad thing necessarily. I do, however, advise discretion. Keep in mind that most people in our culture freak out when they discover that young people actually have a sex live. To that I would add that you probably know that your 19year old playmate is technically breaking the law by cavorting with you, a minor, right? He could get in a shit-load of trouble for bumping an underage lad like you.

And while there’s just a 3-year difference between you, I’d be willing to guess that your friend is considerably more advanced than you in terms of emotional development. If he isn’t, then there’s something very wrong with him. Just be aware that sometimes a disparity in life experience can be used by the more seasoned partner as a means of manipulating the other. And that’s never a good thing.

In terms of coming out, well that’s best done when you are at ease and comfortable in claiming and owning your sexual identity whatever it might be. Since sexuality is often a very fluid thing, there’s no pressing need to self-identify as one thing or another until ya have enough life experience under your belt to be able to say for sure. And even then you may find that your heart…or your dick will lead you elsewhere. Remember what I said about life throwing you a curve ball just as soon as ya think ya have it all figured out? Well it’s true in this regard too.

Finally, and this is the only absolute I have for you today. If you’re old enough to fuck, you’re old enough to know all about safe sex. And not just know about it, but practice it too. If you’re not using condoms, then you’re just a dumb kid with rocks in his head. And I really want to believe that’s not the case with you. Simply put, HIV and other sexually transmitted infections won’t make an exception for you because of your age. Also, you’ll never come to grips with grown-up emotions and all they entail if you’re just a dumb kid with rocks in his head, if ya catch my drift.

Human Rights + Sexual Rights = Sexual Freedom

On this the first annual National Sexual Freedom Day, sponsored by The Woodhull Freedom Foundation, I’d like to propose something quite radical. I suggest that our sexual freedoms, here in the United States, are intricately linked to universal sexual rights. And I contend that the notion of universal sexual rights is at its core a respect for human rights and human dignity.

In a world wracked by poverty, disease and war; where we threaten our very existence with climate altering pollution, nuclear proliferation and extreme population growth; is there room to talk about human rights that include sexual rights and sexual freedom?

I emphatically answer yes! In fact, I assert that sexual inequality and oppression is at the heart of many of the world’s problems. I contend that trying to address human rights without including the essential component of sexual rights and sexual freedom is ultimately doomed to failure.

An absence of sexual rights and sexual freedom leads to domestic and societal violence; human trafficking; suicide; a rise in Sexually Transmitted Infections (STIs); unplanned pregnancies, abortion, and sexual dysfunction.

You know how we are always being encouraged to Think Globally and Act Locally? Well while we busy ourselves securing and celebrating our sexual rights here in this nation, I think we’d do well to focus some of our attention on how our struggle binds us to the rest of the human community.

I offer three examples of what I’m talking about. I invite you to consider how a myopic sexual rights and sexual freedom agenda, divorced from the overarching issues of human, economic and social rights, can be ineffectual and even counterproductive.

In 2008 the research community was all aflutter about ‘conclusive’ evidence linking HIV transmission and uncircumcised males. While I’m certainly not ready to take this data on face value, let’s just say, for the sake of discussion, that the link is conclusive. A massive campaign of circumcision was proposed as the best means of HIV prevention. The medical community would descend on epicenters of the disease, scalpels in hand; ready to eliminate the offending foreskins from every male in sight, young and old.

But wait, there’s a problem. Most HIV/AIDS epicenters are in underdeveloped countries. In these places, access to enough clean water to drink or attend to even the most basic personal hygiene, like daily cleaning under one’s foreskin, remains an enormous chronic problem. Without first addressing the problem of unfettered access to clean water and adequate sanitation, which according to The United Nations is a basic human right, further disease prevention efforts are doomed.

I mean, what are the chances that surgical intervention would succeed—one that would involve significant and sophisticated aftercare—if there is not even enough clean water for drinking and bathing?

These well-meaning medical personnel suggest imposing a strategy that not only works against nature—our foreskins do have a purpose after all: a healthy prepuce is a natural deterrent to infection. But this intervention would also violate long-held cultural and societal norms—circumcision is abhorrent to many of these same cultures. Wouldn’t this proposed prevention effort to stem the tide actually make matters worse?

Indentured sex work is another indicator of how human rights, sexual rights and sexual freedom are intertwined. Until the economic and educational opportunities for women throughout the world improve—which is a basic human right according to The United Nations—women will remain chattel. Families in economically depressed areas of the world will continue to be pressured to sell their daughters (and sons) simply to subsist.

Closing brothels and stigmatizing prostitutes overlooks the more pressing human rights concerns at play here. Sex is a commodity because there is a voracious market. Men from developed nations descend on the populations of less developed nations to satisfy sexual proclivities with partners they are prohibited from enjoying in their own country. Young women (and boys) in developing countries are viewed as exploitable and disposable, because they don’t have the same civil protections afforded their peers in the developed world. And runaway population growth in countries that deprive their women and girls access to education and contraception inevitably creates a never-ending supply of hapless replacements.

Addressing the endemic gender inequality in many societies is key. Equal access to education and economic resources must come before, or at least hand in hand with any serious sexual liberation effort.

Finally, people in the developed world enjoy a certain level of affluence and economic stability which allows them to indulge in sex recreationally. Thanks to effective birth control methods we can ignore the procreative aspects of sex and replace it with a means of expressing a myriad of other human needs. Not least among these are status, self-esteem and self-expression.

If we’re trying to prove something to ourselves, or others, by the way we conduct our sexual lives, simple prohibitions against certain sex practices won’t work. If I’m convinced that unprotected sex with multiple partners and sharing bodily fluids is edgy, cool fun, without serious consequence, as it’s portrayed in porn; I will be more likely to express myself the same way. This is especially true for young people who are already feeling invincible.

Case in point: there has been a startling uptick in seroconversions among young people, particularly gay men, which indicates that disease prevention efforts, even in the world’s most affluent societies, are simply not up to the task. It’s not that there is a scarcity of resources, quite the contrary. It is more likely that these efforts are not connected to a fundamental understanding of the role sexuality plays in the general population. I believe that sexual expression and sexual pleasure are the overarching issues here. These too are fundamental human rights.

No amount of safer sex proselytizing is going to prevail unless and until we look at why and how we express ourselves sexually. As we unravel this complex jumble of motivations and behaviors, effective prevention strategies will manifest themselves clearly. We must develop a sex-positive message; one that celebrates sexuality, builds self-esteem and counteracts the prevailing media messages of sex with no consequences.

National Sexual Freedom Day brings into focus the micro-strategies needed to combat a macro problem. But it also shows that we cannot work for and celebrate sexual freedom in a vacuum. It’s imperative that we see how global health and wellbeing is completely dependent on basic human rights, including sexual rights that include gender and reproductive rights, the elimination of sexual exploitation and the freedom of sexual expression.
